ABSTRACT: To encourage doctors to practice more preventive medicine and make more efficient use of specialty services, managed care organizations and medical groups are experimenting with alternatives to traditional capitation. Variations include paying primary care physicians fee-for-service while capitating specialists, and paying specialists a global fee for all the care a patient may need from a given referral. These payment mechanisms are supposed to offset incentives inherent in capitated systems to avoid seeing patients or to refer them indiscriminately by selectively redistributing physician risk, rewarding primary care doctors for doing more and specialists for managing cases conservatively. However, questions remain about the workability of some of these approaches and whether they will lower physician compensation by limiting the potential rewards of capitation.
