I would like to know what will be the best way to translate this phrase from American English into Colombian Spanish. This is for the translation of a death certificate and a letter from a funerary home located in The United States of America.

This is to state and confirm that Mrs.XXXXX who passed away in 12-XXx-2014,was interred in East Shadowlawn Memorial Gardens located at....xxxxxxxxxxxx her interment space is located at Garden of Calvary,lot 30,space G.
