Estoy traduciendo un estudio titulado "Hypersensitivity Reactions to i.v. Melphalan during Treatment of Multiple Myeloma: Cancer and Leukemia Group B Experience".

"Melphalan (L-PAM) has long been used as a first-line drug in the treatment of multiple myeloma. In several large series reporting the use of oral L-PAM, no allergic reactions were reported except for the rare occurrence of a skin rash (1-4). Between 1971 and 1977, i.v. L-PAM was used in three completed randomized studies of multiple myeloma sponsored by Cancer and Leukemia Group B (CALGB) (protocol Nos. 7161, 7261, and 7461). Informed consent was obtained prior to patient entry in each study. Figure 1 shows the treatment programs for each of the three studies containing i.v. L-PAM. L.-PAM was initially dissolved in 10% ethanol and diluted to concentration of 10 mg/ml in normal saline and injected slowly (1-2 minutes) directly i.v. or into the tubing of a running i.v. infusion. In all three trials, one group of patients was randomized to receive oral L-PAM (0.15 mg/kg/day for 7 days. with a maintenance dose of 0.05 mg/kg/day after recovery from the **nadir of the peripheral wbc count**) together with an initial 6-10-week course of oral prednisone in tapering doses."

Según Navarro, nadir se refiere a "a la concentración mínima registrada en una serie temporal de determinaciones. En el ámbito médico se usa sobre todo referido a la cifra mínima de linfocitos T CD4 registrada en un paciente con sida antes de iniciar el tratamiento antirretrovírico; en ese caso, considero mucho más claro y preciso decir ‘cifra mínima de linfocitos’ que *nadir de linfocitos*"

Puede que aquí se refiere a la cifra mínima de linfocitos después de un tratamiento con melfalán, pero quisiera corroborarlo, además de saber a qué se refiere "peripheral" en este contexto.
