This is an entry on a registration form with a translation agency. I don't really need a translation for it, if someone could just kindly explain it.

SSN - Social Security Number / TIN - Taxpayer Identification Number

This is a form from a US translation agency?

These numbers are required by the US Internal Revenue Service. Each individual has a social security number, if you are incorporated, you have a taxpayer identification number. In either case, it enables Uncle Sam to track you for tax purposes.
