On a table that lists all of the medications taken by a patient during a given period, the term "hors formulaire" comes up in the information that accompanies the drug Nexium, in the same space as the place where the side effects were listed for the other medications. My assumption is that there is no product insert, and that this term indicates there is no information to list on the side effects.

Can anyone confirm this for me?

In your text, “formulaire” probably refers to the Quebec provincial formulary, a long list of drugs covered by the public prescription drug insurance plan. “Off-formulary” would describe a drug that is not in the formulary. Curiously, Nexium is in the formulary. Another possibility, perhaps, is that Nexium was used for an indication not covered by the plan. Would either one of these possibilities work in your context? There are a good number of examples of “off-formulary” (in the context of drugs) on the Internet.
Example sentence:

Consideration should be given to allowing patients to access off-formulary drugs if, in the opinion of the attending physician, the recommended product is not the right choice for them.

agree Michael Barnett : Do you have a ref to the Quebec formulary? I would be surprised if it were on it, since Nexium costs 4 times as much as some competing PPIs.// So it is! Good work! It is possible that the patient did not qualify for this drug even though it's on the list.

agree Jean-Louis S. : The problem is that "formulary" is "liste de médicaments" in French. "Formulaire" is something different. See [http://www.cnrtl.fr/definition/formulaire], for instance, or previous KudoZ.// then formulary it is.

In Quebec, the provincial formulary (Liste de médicaments) is commonly referred to in French as “le formulaire”.

off-label

This may be a US term.

It indicates that a drug was given for a non-approved reason, possibly making use of the side effects. For instance, Viagra was originally for something else (heart medication?) and it's more common use was originally a side effect. If it was given for that reason, it was "off-label". Of course, that situation has changed!

I have taken Nexium and there definitely is an insert.
