From this document (http://www.pmda.go.jp/files/000229507.pdf), it appears that "薬機品安発" in the top right corresponds to "独立行政法人医薬品医療機器総合機構医療機器品質管理・安全対策部長" in the list of department heads below.
According to the PMDA's lists of its departments in Japanese (https://www.pmda.go.jp/files/000227073.pdf) and English (https://www.pmda.go.jp/files/000227298.pdf), 医療機器品質管理・安全対策部 corresponds to "Office of Manufacturing Quality and Vigilance for Medical Devices".
Using other translations of official notifications on the PMDA's website as a guide (e.g. https://www.pmda.go.jp/files/000229118.pdf; full list available https://www.pmda.go.jp/english/review-services/regulatory-in... official translations follow a format of "abbreviation of organisation name/abbreviation of department name + Notification No. + number". I haven't found any English translations referencing this department and therefore see no official abbreviation — MQVMD is just my suggestion.
