Can't help with Mle., but... shouldn't it be "vesicae urinariae"?

Metastasis (for M) , lymphoglandulae (for l), evidens or existo (for e)

My guess it should be metastasis because there is the description of lung's obstruction. But "obstruction" is an English word (but its origin is Latin: ob-struo/before - built/. The term " pulmonum" is used in English and exists in med.dictionary (and "vesica, urinary, St/age/" also).
Sorry for guessing but it is the best I can do without the additional information.
