You'll often see words to this effect in notarial "testimonios", i.e., official transcripts of entries in notary's registers.
Obviously, this is a slightly different context, but the idea remains the same.
Not having the original document before me, I can't be sure exactly how to word it, but the idea is this:

"This is a faithful copy of the relevant part/section/passage in the original document/entry."
