On a patient's discharge sheet, at the moment I have "referred symptomatic improvement" but I'm not convinced that's correct as it pops up a total of 0 times when googled.

"Actualmente curando con buena evolución clínica, mejoría sintomática referida, por lo que se decide egreso a su domicilio."

referir Can mean to "refer (to)" -- OR "report" in a medical context. The online Reverso dictionary gives "contar" as a synonym (to tell, recount) and if you put "el paciente refiere que" into Google translate, the first option it shows is "the patient reports that".

improved symptoms

You could add "patient reports improved symptoms" because I think "referida" means the patient has reported an improvement in his/her symptoms, but I think that "improved symptoms" would suffice.

agree Alvaro Aliaga : I prefer "patient reports improved symptoms" as it seems to be one of the last notes and clarifying that the improvement is reported by the patient is important, as opposed to just observed by the treating physician.

I agree with Emma that "referida" probably means reported by the patient and I likewise think it's not necessary.

Both Google and I prefer "improvement in/of symptoms" to "improved symptoms".
