Spanish to English Medical Medical (general) Patient chart/progress notes

This comes from a list of health history issues for a deceased patient, from their final medical chart. I know what it means, I'm just trying to think of the best way to say it in "medicalese". Just to give an idea, some other items from the list include "Left vascular ischemic attack with sequelae: right FBC hemiparesis; neurogenic bladder disorder (Foley kit user)".

I'm thinking "chronic smoker who quit", but that just doesn't sound very elegant (medical) to me. I can't think of any adjectives in American English for quitting smoking. (American English is the target language.)

I'm not sure of the country of origin, but given the generic medical nature of the language, I don't think it matters.

Any suggestions?
