This appears after "Date filed with Registrar." Any ideas on how to correctly render this for a Panamanian audience?

Estoy traduciendo una partida de nacimiento en la que sale el término. Para futuras referencias, pienso que debería ser "fecha de rectificación o fecha de modificación" porque lo que se rectifica no es la fecha, sino que se trata de la fecha en la que se realizó la rectificación.

I don't think you need Panamanian Spanish for this. Just Latin American Spanish.
