Medical grounds for discharge - normally a physician once he/ she establishes a physician/ patient relationship, he/she cannot abandon the patient without showing the reason/ grounds for discharge.

I would need more context to be 100% sure that this is the case, but I am quite sure that this is the intent.

medical reason or purpose

bit that sounds strange for discharge unless it's a transfer.
