I'm confused. Is there a photograph hanging above the entrance to the hospital? Or are we talking about a photograph of a motif hanging above the entrance?

I believe that the picture may be dated 1902, but more likely it presents entrance to the hospital and date 1902 above it (placing date above the door was quite common practice).
