I read the original as saying that it's the research subjects that are healthy or diseased. (Presumably, certain metabolic processes are abnormal in a diseased subject, but I doubt all of them are; you could conceivably study metabolic processes in a diseased subject and find that all of the ones you are looking at are normal.)

I think it's as simple as that - we are not directly talking here about human health itself but, more specifically, about studying normal and abnormal metabolic processes.
