If you translate "tight" not as "fest", "dicht" oder "eng" but as "streng" you cover all aspects mentioned in MagRaWa's comment that I fully agree with: more frequent control/measuring and monitoring the level of blood pressure. It expresses it complexly. I wonder why my precessors seem to have trouble with finding one word.

wenn mam weiß, wie die Blutdruckmessung funktioniert, ist es klar, dass die Manschette nur zum Aufbau von einem notwendigen Druck für die Messung da ist. Wie eng, spielt ab der für die Messung notwendigen Grenze keine Rolle mehr.

Zweitens denke ich, dass die nummerische Angabe in der Klammer ein Hinweis darauf sein könnte, was mit "tight" genau gemeint ist. Die modernen digitalen Geräte messen in ganzen Zahlen, vielleicht geht es hier um eine noch genauere Messung. Leider weiß ich selber nicht genau, was die Angabe in Klammer bedeutet. Sorry.

Deswegen, würde ich die Übersetzung "genauere Kontrolle/Messung" vorschlagen.

Mit modernen Medikamenten kann man den Blutdruck präzise steuern, nicht nur kontrollieren. Die Angaben in Klammern sind alternative Abstände zwischen den Grenzwerten des Zielbereichs. Sobald ein Grenzwert überschritten wird, wird eine Maßnahme ergriffen, um gegenzusteuern.
