There are two terms in my document following closely upon each other:
"Poistitel je povinny:
"a. zasielat poistnikovi najneskor do 18. dna kalendarneho mesiaca nasledujuceho po uplynuti poistneho obdobia, 'Vykaz poistnych udalosti za mesiac...", ktoreho vzor je v prilohe c. 3 k tejto zmluve,
b.,
And almost exactly the same sentence, only with ''Vykaz poistneho" in place of 'Vykaz poistnych'.
The first is 'insured events', but what is the second? Report on premiums?
My rough start:
The insurer must:
5.1 send to the insured party at the latest by the 18th day of the calendar month following the end of the insured period �Report on insured events for the month of ��, a template for which report is contained in Attachment 2 of this contract...

Comment: "Apologies for the delay in closing this question: the next 17 pages of this microscopic-font faxed contract clarified what was happening in these first three pages.
Since the context has much on 'poistne', or premiums, I decided to go with 'premiums' here too. Poistne obdobia = premium period... poistne plnenia= indemnification. Poisteny = insured party.
And so on.
Need a magnifying glass now.
And that's just to find my toothbrush."

Statement of account
I wouldn't bother with literal translation. "Statement of account" - that's it. The whole thing though should read as: "The insurer must:
1) send a statemnte of account to the insured by the 18th day of the month after which the policy ended.
2) detail events occuring during duration of the policy in the format in attachement 2
