I have translated this as 'Acute renal insufficiency 2nd A toxicity due to aminoglycosides'. I'm not happy about the '2nd A' as I'm not sure where it fits in. Many thanks for your help!

I agree with the rest of your translation. 2a is an abbreviation for 'secondary to' (due to, as a result of)

On further thought, I would re-word the sentence slightly:

"Acute renal insufficiency secondary to aminoglycoside toxicity."

acute kidney failure as a result of toxicity due to aminoglycosides

I would phrase the first part as "acute kidney failure" although "renal" is certainly understandable

agree Joseph Tein : Hi, Mike ... this is correct also. The last doctor I spoke to - about 20 min. ago - says that 'renal insufficiency' and 'kidney failure' are essentially interchangeable. Saludos to you and your dad :)
