"Do not operate" tag
It seems all the answers above are correct. But I have never seen "Operation Prohibition" tag OR "Lockout" tag on any machine. I will go for "Do not Operate" tag which is widely used term for industrial purposes.
(Just search the term on Google, you will find the tag images as well)
