Context:

Paciente hombre de XX años, ingresa por neumotórax.

ANTECEDENTES
- Diabetes mellitus secundaria a corticoides.
- Dislipemia.
- Neumopatía intersticial secundaria a síndrome antisintetasa con anticuerpos anti-Jo 1 positivos diagnosticado en 2001. Desde el 2009 con empeoramiento progresivo de la patología pulmonar.
- Transplante unipulmonar izquierdo en Enero de 2011: Episodio de rechazo agudo A 2 tras a los 2 meses. Se incrementó dosis de esteroides, y probablemente dicho episodio se asoció a niveles bajos de IS (en aquel momento con FK de 4.8). Se realizó control con FBS en Agosto con nueva BTB que mostró solo neumonitis linfocitaria. Actualmente en dosis decreciente de corticoides 24 mg. BAL – cultivos negativos. Neutrofilia en BAL de mayo, se incrementó Azitromicina y se cambió por intolerancia digestiva IBP de omeprazol a pantecta. En FBS de Agosto PMN 1%. Actualmente en tratamiento con microfenolato, tacrolimus y corticoides. En ensayo clínico de CyAs inhalada.

In this context, "IS" is probably "inmunosupresores". The author suggest the patient's rejection was due to low levels of anti-rejection drugs in the bloodstream.

"FK" is a common abbreviation for tacrolimus. (A trough level of 4.8 would indeed be subtherapeutic.)

IS low levels

where IS (in English - Immunosuppression). As in this case I will go for the same term: Inmunosupresión.
No literature available in Spanish to conclude, but if you review / Tacrolimus (FK-506) (PROGRAF®), some links appear.

I will recommend to wait if some one else has a better answer.
