The patient presented to us following a witnessed seizure and fall with C2H50H on board. He dropped his CGS from 15 to 3 following recovery from seizure. CT brain showed a large right-sided extradural hematoma,
biconvex hyperdense collection of blood pushing the right cerebral hemisphere medially and early subfalcine and transtentorial herniation.
What do they mean by " fall with C2H50H on board" ?

fall while under the influence of alcohol
"C2H5OH" is the chemical formula of alcohol. ER physicians and emergency personnel will also often refer to it as "EtOH", for "ethanol".
"on board" is also medical jargon for having something "in your system", usually following a dose and medication (e.g. "XXX mg of YYY on board").

C2H5OH is the formula for ethanol, so presumably the patient had been drinking and had a seizure and fell down. Whether or not the alcohol contributed to either the seizure or the fall isn't clear.
