For XXX, planning is a continual process and involves knowing what her team can do now, growing their skills for future opportunities and hiring for specific needs on her team.
Coaching is a part of XXX’s daily activities. This has helped her to identify what people are passionate about and what their goals are. She is able to ***connect*** development opportunities that will motivate and build skills towards future growth.

Does it mean "connect the opportunities to the (right) team members"?

It is indeed a strange way to put it. I think what it means is that XXX tries to connect development opportunities to the employees' goals.

I think this is an unusual use of "connect." If you could provide the next sentence or two from the text, the meaning might be more clear. At the moment, to my mind, it's really a matter of guessing what the author might mean. More context (more of the author's words, in this case) would hopefully help.

associate - make a logical or causal connection
combine - put or add together

My option is to associate developments oppotunities with skills building.
