Typo!! I meant to say 'opinion'. Another way to say it would be 'where I consulted Dr. x', but it doesn't quite convey the idea of bringing in a specialist, even if he was right there.

Entiendo que el paciente fué derivado a terapia intensiva ante la posibilidad de un fallo respiratorio. No es claro si se lo derivó a ese médico en particular o a quien estuviere a cargo de Terpia Intensiva, de ahí los paréntesis.

: "it" sounds awkward here as the sentence clearly refers to a patient, not a case. And I think that "me" in the source refers to the person who is writing the report.
