: I definately agree that the intent is as David expressed, however I wonder if maybe a word is missing? על שמי כחוק "a personal perscription legally made out to/perscribed to my name

Well, if this is a condition, then presumably it isn't in the first person. I read שמי as שֵׁמִי.
