Думаю, swell можно перевести как "закупоривать", а debris - как "выделения" (при инфекции). Про формулировании ответа посоветовался с доктором))

Думаю, что между "swollen" и "debris" есть запятая (фигурально выражаясь). Т.е. "когда it is swollen, тогда debris следует удалять"... А "it" относится к слову "canal", стоящему перед "when".
