It may sound nasty -- when taken out of context -- but so does "Entlassungsmanagement"!
In fact the term is used extensively in health management education, as over 1,000 google hits proves.
Incidentally, "discharge management" is also used in the context of "waste and discharge management" (i.e. waste water discharge management) -- which is about as nasty as it gets!

"Discharge management" sound nasty, particularly in a medical context!

discharge management

I don't agree with Nigel regarding "discharge management" - it's a term I've often heard and which is widely used on the internet. This would be the best option here, in my opinion.


"Arrangements for management of discharge need radical overhaul in many hospitals. In the Committee’s view, best practice involves a multi-agency team, with clear links to all key points in the patient’s journey, actively managing all aspects of the discharge process. The leader of this team should be jointly appointed by the NHS and councils with social services responsibilities. The discharge management process should be proactive rather than reactive."

hospital discharge planning

WHAT IS HOSPITAL DISCHARGE PLANNING?

Hospital discharge planning is a service to assist patients arrange care needed following a hospital stay. Discharge planners help arrange services including home care, nursing home care, rehabilitative care, out-patient medical treatment and other help. Hospital discharge planning is usually handled by the hospital’s Social Services Department.

Might work well here since the original text is about transferring patients to a rehabilitation facility.
