I've asked some English-speaking friends and they said they never heard of the expression "against signature" which Multitran proposes as translation for под подпись

Можно попробовать такой вариант. Например, если речь идет о "пособии" для сотрудников, где расписаны политика компании, дресс-код, правила относительно отпуска и т.д., то часто это действительно отдельная страница с текстом в духе "прочитал и ознакомлен", где надо поставить подпись. Может быть, для вашего документа тоже подойдет acknowledgement form.
Можно еще попробовать как-то поиграться с acknowledge the document by signing (signature).
