Discussion

TonyTK Apr 30, 2021:
While "Träger" and "Betreiber"are sometimes ... ... two different entities, they're very often identical in the care/hospital sector. As "Anstaltsträger" can also be private entities, you could go with something like "operator of the institution". In this specific case, you might also get away with "hospital operator".

AUT: public-sector sponsor of the institution

public-sector sponsor e.g. die Stadt Wien of the institution e.g. das AKH or die Klinik Hietzing bzw. das Lainzer Spital of old where the medical orderlies / Krankenpfleger had been keen on discussing famous British and Irish billiards- and snooker-players during my surgical op.: no, not a head transplant.

Anstaltslast> liability assumed by the public-sector owner : govt. authority or municipality If this is referring, for instance, to the WGKK - Wiener Gebietskrankenkasse, then literal translations like 'instiutional carrier' wouln't be very helpful.
Example sentence:

Die Rechtsstellung der Sparkassen und der kommunalen Anstaltsträger", Juli 2003 pwcv.de The legal position of the savings banks and the local institutional authorities", July 2003

in Kenntnis des Ärztestandes betreibe jedoch der Anstaltsträger *Stadt Wien* das AKH in einem Ausmaß, das etwa 231 Ärzte erfordern würde

4 KudoZ points awarded for this answer. Comment: "Thank you for the info. At the end of the day, I actually opted for TonyTK's "hospital (clinic) operator" as this piece of information has to stay as simple and concise as possible on the patient info form, and to be honest is not actually crucial for the intended readership (in fact, will most likely not be read by 95% of those who sign the form!)"
