Spanish term or phrase: ACTA DE LOCALIZACION DE NIÑAS, NIÑOS Y ADOLESCENTES
I am translating a series of documents related to a divorce from El Salvador. Among them is this "Acta de Localización". It is a document indicating the conditions in which the minor son of the spouses seeking divorce was found, when and where he was found, etc.

Thanks in advance for your help!

English translation:Report of location of children and adolescents
Explanation:
Apparently a police report. "Girls and boy" can be condensed to "children" imo

I suspect philgoddard might be along soon to suggest "children and adolescents location report"

And, despite the plural, it is perhaps more sensible in English to use the singular
"Child and adolescent location report"

Or even simply
"Minor location report"
Still makes sense

Child/adolescent trace report

Explanation:
Improvement on my earlier answer
The report is about an individual so the use of the plural sounds unnatural in English
The Spanish term is too wordy and I think it can be condensed in the way I have suggested here, without any loss of meaning
