German to English Medical Medical (general) Medication dosages

This is in a table in a medical report. The heading is "Zeit / Dosis", and the choices are: 1) "bel. Zeiten", 2) "Standardzeiten", 3) "bis 24 Mal", and 4) "ohne Zeiten". I have a feeling this is going to seem painfully obvious to some, but I haven't been able to come up with anything. My best guess at the moment is "belegten Zeiten", which could mean "documented times", but that doesn't seem terribly different to me than "Standardzeiten", although I guess it could mean times that deviate from the standard times (which are also given in the table, but that are well documented in the record. Any ideas? Thanks in advance.
