je ne vois pas comment traduire "record" dans ce contexte.

"After the investigation is completed, if the report is determined to be “indicated” and if you are determined to be a subject of the report, you have the right to request the Commissioner of New York Office of Children and Family Services to amend (change) the record of the report if you believe that the report is inaccurate. This request must be made by you within 90 days of being notified that the report is indicated.
The subject of the report is the person(s) responsible for causing or allowing to be inflicted injury, abuse or maltreatment to the child (ren)".

Je crois qu'on parle de la caractérisation/l'annotation/la qualification, à la rigueur le "classement" du "rapport" en question (que je soupçonne être en fait un "signalement"), laquelle/lequel est susceptible d'enclencher les mesures de protection appropriées:

"Une fois l'enquête complétée, s'il est établi que le [rapport][signalement] est ["indiqué"]["fondé"] et si vous faites l'objet de ce [rapport], vous avez le droit de demander au Commissaire du New York Office of Children and Family Services de réviser (modifier) [la caractérisation] [le classement] du rapport si vous le croyez inexact...

J'opterais pour "classement" étant donné qu'il me semble qu'on chercher à vulgariser le texte (avec l'annotation amend = change).
