It means is that overseas US military hospitals (outside the continental US, to include Hawaii and Alaska) are intended for military personnel. If they are stationed in Germany, for example, their family members are treated in German hospitals. The military hospitals have only enough staff to treat military personnel. There are exceptions for family members with special needs.

Military Family members.
Family members will not be admitted to an Army MTF when their needs are only for domiciliary or custodial care. Family members may be hospitalized for chronic conditions and nervous, mental, and emotional disorders that require active and definitive treatment. Admission will be according to the order of priority in paragraph 2-3
