This is the term mostly used in the Spanish-speaking health care industry of USA, with some variants. It's also the term used by the Centers for Medicare & Medicaid Services (CMS). The noun "bolsillo" is usually preceded by the possessive adjective "su", according to the context.

I've been translating dozens of health care and Medicare plans for years for the US market. I've seen different options: gastos del propio bolsillo, for example.
As always, it pays to understand what the insurers mean with this expression. An out-of-pocket medical expense means a medical service you pay right away (whether with credit card, HSA account card, cash or check). The key element here is immediacy of payment. For example, a copay is an out-of-pocket expense. When you go to the pharmacy to buy a $30 medicine that your insurer covers 50% of, the difference or $15 you have to pay to take the medicine with you is an out-of-pocket expense.
That's why I prefer to go with gasto médico directo or something along those lines. Another option: gasto médico de pago inmediato.

Por ejemplo, si es marzo y yo recién voy al médico por primera vez en ese año, deberé pagar el copago, más un coseguro (si corresponde), los honorarios de la visita y otros servicios (una radiografía, eco Doppler, cirugía menor, etc.).
5) Algunos hospitales y clínicas cobran una recargo por uso del establecimiento, que puede ser más caro que la propia visita al médico.
En la lista anterior, los puntos 2 y 3 se consideran normalmente gastos directos o desembolsos inmediatos. No son beneficios en absoluto. El cliente que dice que le dejes “out-of-pocket” en inglés es un salame, con perdón a los embutidos. :)
