(completed) power of attorney and/or authorization letter forms

"/" means nothing in terms of legal language. However, "and/or" is a legal term used in contracts. There is even case law regarding the meaning of "and/or" in contracts and such.

"and/or" is necessary here, since a power of attorney differs significantly from an authorization letter.
