Mexican variant to US English. I am translating an affidavit, of sorts, which is a request to give temporary custody of a child to a grandmother for travel to the U.S. to undergo a medical procedure. I am not super-experienced with legal documents. The phrase seems simple enough, but I'm having a heck of a time wording it in English without the translation sounding stupid. The phrase comes at the closing of the document, "Es todo lo que tiene que decir, es por eso que pone conocimiento a esta autoridad de ciertos hechos." Any help with a natural-sounding legalese phrase in English would be greatly appreciated!

Wow, I'm glad this has created so much discussion! But to clear things up: the register is very formal. I said "of sorts" because there is no title on the document. I'm just calling it an affidavit as a general term for a notarized statement. The "appearing parties" signing the document are the maternal and paternal grandmothers. It is concerning the child but these two parties are the ones that are saying "FYI and that's all folks" in my colloquial paraphrasing.
