Source text - French Les maladies cardiovasculaires sont la première cause de mortalité et de handicap dans les pays développés. Elles représentent en France et dans le monde, un enjeu majeur de santé publique.
La maladie coronaire, l’accident vasculaire cérébral ischémique et l’artériopathie oblitérante des membres inférieurs, sont les complications, le plus souvent tardives, de l’athérosclérose, phénomène inflammatoire artériel chronique, induit et entretenu par un excès de cholestérol circulant. Dans ce document, l’ensemble de ces complications sera regroupé sous le terme « cardiovasculaire ».
1. La prévention cardiovasculaire se conçoit à deux niveaux :
- la prévention primaire concerne les patients sans maladie cardiovasculaire ;
- la prévention secondaire concerne les patients qui ont une maladie cardiovasculaire telle que définie ci-dessus.
Les enseignements des grandes études d’intervention ont fait évoluer les bases de la prévention cardiovasculaire par les hypolipémiants sur 5 points :
1. La prévention secondaire s’adresse aux patients qui présentent non seulement des antécédents coronaires, mais aussi d’autres maladies vasculaires athéromateuses avérées : accident vasculaire cérébral/accident ischémique transitoire, artériopathie oblitérante symptomatique des membres inférieurs.
2. Le bénéfice de la prévention cardiovasculaire (primaire et secondaire) par les hypolipémiants n’est pas exclusivement coronaire. Chez les patients à haut risque, une diminution du risque de premier accident vasculaire cérébral est démontrée. Ce bénéfice s’étend à l’ensemble des événements cardiovasculaires.
3. La prévention cardiovasculaire a des effets démontrés dans de nouveaux groupes de patients parmi les sujets âgés de 70 à 80 ans, les femmes ménopausées, les patients hypertendus, les diabétiques de type 2 ou les patients ayant des antécédents vasculaires personnels.
4. Les patients ayant fait un accident coronaire doivent bénéficier du traitement préventif au décours de l’accident aigu.
5. La réduction du risque cardiovasculaire dépend de l’abaissement des concentrations de LDL- cholestérol. Elle est également observée chez les sujets à risque ayant des concentrations de LDL- cholestérol proches des valeurs moyennes de la population générale.
Translation - English Cardiovascular diseases are the leading cause of mortality and disability in developped countries. In France and in the rest of the world, this constitutes a major public health issue.
Coronary disease, ischemic stroke and occlusive arterial disease of the lower limbs are, more often than not, late-onset complications of arteroclerosis, a chronic arterial inflammation, resulting from and maintained by an excess of cholesterol circulating in the blood. In this document, all these complications will be defined as "cardiovascular".
1. Cardiovascular prevention intervenes on two levels :
- primary prevention on patients without cardiovascular disease
- secondary prevention on patients with a cardiovascular disease as defined above.
The teachings of great studies of intervention have impacted on the basics of cardiovascular prevention, using hypolipidaemics, on 5 points.
1. Secondary prevention is aimed at patients with a history of not only coronary but also other proven artherosclerotic diseases : stroke/ transient ischemic accident, symptomatic arterial occlusive disease of the lower limbs.
2.The benefits of cardiovascular prevention (primary or secondary) using hypolipidaemics are not exclusively coronary. In high-risk patients, a decrease of first stroke is proven. This benefit extends to all cardiovascular events.
3. The effects of cardiovascular prevention have been proven on new groups of patients among subjects aged 70 to 80 years, post-menopausal women, hypertensive patients, type 2 diabetics or patients with a personal vascular history.
4. Patients having had a coronary accident must receive preventive treatment during the acute accident.
5. The reduction of cardiovascular risk depends on lowering the levels of LDL-cholesterol. It is also observed in subjects at risk with levels of LDL-cholesterol that are close to average levels in the general population.
