you can translate : Those correspondences must be sent before 03/09/09 12 am, using the requisition form attached.

This would be the English term if it's an actual piece of paper - it's a form with areas for you to fill in sender details, addressee details, date, subject, plus an area for the text.
Slightly different if it's on the internet, where it's more often called a contact form, but the idea is the same.

Ormiston - As it's referring to a "demande" then, yes, I think it could well be called an "application form" in this particular case.

easily done, a permanent danger living abroad. I can't seem to be able to add this under 'clarification' - but I was wondering (as shares are being bought/sold here) if it's slightly like 'vente par correspondance', i.e. by letter & it is simply a 'mailed application' i.e. simply an 'application form' (!) |
