居所地 means the address of one's place of abode, where he/she currently lives.
住所地 means his/her domicile, a registered location where he/she may or may not live.

Since this is a document I think "address of abode" is the most appropriate translation.

The difference between 住所地 and 居所地 are whether or not you register as your abode address at city (or ward) hall. If you live there continuously, but you've got another place to live and registered as a real address of your abode, the latter (registered one) is 住所地 and the place you use as your living (and could be also business) place, it's 居所地.
For the people who work from home (or his/her place is SOHO), 住所地 and 居所地 are the same address.
