Explanation:
Due to the statement, "nicht vollzogen", it seems likely that TR is a procedure -- which in this case was not completed.

Three of the most common heart tests are ECG (also called EKG), chest X-ray, and blood tests. I believe TR must stand for "Thorax-Röntgen", which is indeed a chest X-ray.

You might be tempted to translate this as "thoracic radiography", which does occur in English. Note, however, that in human medicine the preferred English term seems to be "chest X-ray". Search results for "thoracic radiography" appear largely to relate to veterinary medicine (i.e., animal medicine).

In the context of an ECG, there is also such a thing as a "T-R interval". However, I do not see how it could be reconciled with "nicht vollzogen".
