Likely to mean a first mortgage, but NB the doctrine of tacking (FRE > adjonction: over same land vs. consolidation over different plots) and that could allow a mortgage advance to be added on to earlier one by the same mortgage lender and 'leapfrog' the mortgage applied for.
Example sentence:
