MSI-234 in Drug-Naїve Patients (Initial Therapy): The recommended starting dose of MSI-234 as initial therapy is 2 mg/500 mg administered once or twice daily. The dose of MSI-234 may be increased in increments of 2 mg/500 mg per day to a maximum of 8 mg/2,000 mg per day given in divided doses if patients are not adequately controlled after 4 weeks.
In stress situations (e.g. trauma, surgery, infections) blood glucose regulation may deteriorate, and a temporary change in dose of antidiabetic medication or a change to insulin may be necessary to maintain good metabolic control.
The drug has been reported to cross the human placenta and to be detectable in foetal tissues. There are no adequate data to support the use of MSI-234 during pregnancy in humans. The use of insulin is generally recommended for patients with diabetes during pregnancy.
In a retrospective analysis of data from pooled clinical studies, the overall incidence of events typically associated with cardiac ischaemia and angina was higher for elekira containing regimens, 1.99% versus comparators, 1.51% [Hazard ratio 1.31 (95% confidence interval 1.01 - 1.70)].
Post marketing reports have been received rarely for elekira as monotherapy and in combination with other antidiabetic agents. It is known that the risk of developing heart failure is increased substantially in diabetic subjects compared to non-diabetic subjects.
HYPERTENSION
The safety and efficacy of KRD-123 as initial therapy for severe hypertension (defined as a mean seated diastolic blood pressure (SeDBP) ³110 mmHg confirmed on two separate occasions off all antihypertensive therapy) was studied in a seven-week, double-blind, randomized, multicenter study. Patients were randomized to either carvedilol and hydrochlorothiazide (150/12.5 mg) or to carvedilol (150 mg) once daily and followed for blood pressure response. These initial study regimens were increased at one week to carvedilol 300 mg/HCTZ 25 mg or to carvedilol300 mg, respectively. DEFINITION
O-β-D-Galactopyranosyl-(1→4)-α-D-glucopyranose monohydrate.
CHARACTERS
Appearance: white or almost white, crystalline powder.
Solubility: freely but slowly soluble in water, practically insoluble in ethanol (96 per cent).
IDENTIFICATION
First identification: A, D.
Second identification: B, C, D.
A. Infrared absorption spectrophotometry (2.2.24).
Comparison: lactose CRS.
B. Thin-layer chromatography (2.2.27).
Solvent mixture: water R, methanol R (2:3 V/V).
Test solution. Dissolve 10 mg of the substance to be examined in the solvent mixture and dilute to 20 ml with the solvent mixture.
Reference solution (a). Dissolve 10 mg of lactose CRS in the solvent mixture and dilute to 20 ml with the solvent mixture.
Reference solution (b). Dissolve 10 mg of fructose CRS, 10 mg of glucose CRS, 10 mg of lactose CRS and 10 mg of sucrose CRS in the solvent mixture and dilute to 20 ml with the solvent mixture.
Plate: TLC silica gel G plate R.
Mobile phase: water R, methanol R, glacial acetic acid R, ethylene chloride R (10:15:25:50 V/V/V/V); measure the volumes accurately, as a slight excess of water produces cloudiness.
Application: 2 µl; thoroughly dry the starting points.
Development A: over a path of 15 cm.
Drying A: in a current of warm air.
Development B: immediately, over a path of 15 cm, after renewing the mobile phase.
Drying B: in a current of warm air.
Detection: spray with a solution of 0.5 g of thymol R in a mixture of 5 ml of sulphuric acid R and 95 ml of ethanol (96 per cent) R; heat at 130 °C for 10 min.
System suitability: reference solution (b):
— the chromatogram shows 4 clearly separated spots.
Results: the principal spot in the chromatogram obtained with the test solution is similar in position, colour and size to the principal spot in the chromatogram obtained with reference solution (a).
C. Dissolve 0.25 g in 5 ml of water R. Add 5 ml of ammonia R and heat in a water-bath at 80 °C for 10 min. A red colour develops.
D. Water (see Tests).
TESTS
Appearance of solution. The solution is clear (2.2.1) and not more intensely coloured than reference solution BY7 (2.2.2, Method II).
Dissolve 1.0 g in boiling water R and dilute to 10 ml with the same solvent.
Acidity or alkalinity. Dissolve 6.0 g by heating in 25 ml of carbon dioxide-free water R, cool and add 0.3 ml of phenolphthalein solution R. The solution is colourless. Not more than 0.4 ml of 0.1 M sodium hydroxide is required to change the colour of the indicator to pink.
Specific optical rotation (2.2.7): + 54.4 to + 55.9 (anhydrous substance).
Dissolve 10.0 g in 80 ml of water R, heating to 50 °C. Allow to cool and add 0.2 ml of dilute ammonia R1. Allow to stand for 30 min and dilute to 100.0 ml with water R.
Absorbance (2.2.25).
Test solution (a). Dissolve 1.0 g in boiling water R and dilute to 10.0 ml with the same solvent.
Test solution (b). Dilute 1.0 ml of test solution (a) to 10.0 ml with water R.
Spectral range: 400 nm for test solution (a) and 210-300 nm for test solution (b).
Results:
— at 400 nm: maximum 0.04 for test solution (a);
— from 210 nm to 220 nm: maximum 0.25 for test solution (b);
— from 270 nm to 300 nm: maximum 0.07 for test solution (b).
Heavy metals (2.4.8): maximum 5 ppm.
Dissolve 4.0 g in water R with warming, add 1 ml of 0.1 M hydrochloric acid and dilute to 20 ml with water R. 12 ml of the solution complies with test A. Prepare the reference solution using lead standard solution (1 ppm Pb) R.
Water (2.5.12): 4.5 per cent to 5.5 per cent, determined on 0.50 g, using a mixture of 1 volume of formamide R and 2 volumes of methanol R as the solvent.
Sulphated ash ►(2.4.14): maximum 0.1 per cent, determined on 1.0 g.◄
Microbial contamination. Total viable aerobic count (2.6.12) not more than 102 micro-organisms per gram, determined by plate-count. It complies with the test for Escherichia coli (2.6.13).
STORAGE
In an airtight container.
FUNCTIONALITY-RELATED CHARACTERISTICS

Fase Móvil: agua R, metanol R, ácido acético glacial, cloruro de etileno R (10:15:25:50 V/V/V/V); medir los volúmenes con precisión, ya que un leve exceso de agua produce vapor.
Aplicación: 2 µl; secar completamente los puntos de inicio.
Desarollo A: sobre un trayecto de 15 cm.
Secado A: en una corriente de aire cálido.
Desarrollo B: inmediatamente sobre un trayecto de 15 cm, después de renovar la fase móvil.
Secado B: en una corriente de aire cálido.
Detección: rociar con una solución de 0.5 g de timol R en una mezcla de 5 ml de ácido sulfúrico y 95 ml de etanol (96 por ciento); caliente a130 °C por 10 min.
Compatibilidad del sistema: Solución de referencia (b):
— El cromatograma muestra 4 puntos claramente separados.
Resultados: el punto principal en el cromatograma obtenido con la solución de prueba es similar en ubicación, color y tamaño al punto principal en el cromatograma obtenido con la solución de referencia (a).
C. Disolver 0.25 g en 5 ml de aguar R. Añadir 5 ml de amoniaco R caliente a baño maría a 80 °C por 10 min. Se produce un color rojo.

PRUEBAS
Apariencia de la solución. La solución es transparente (2.2.1) y no más intensamente coloreada que la solución de referencia BY7 (2.2.2, Método II).
Disolver 1.0 g en agua R hirviendo y diluya a 10 ml con el mismo solvente.
Acidez o alcalinidad. Disolver 6.0 g calentándolo en 25 ml de agua libre de dióxido de carbono R, enfriar y añadir 0.3 ml de solución de fenolftaleina R. La solución es incolora. No más de 0.4 ml de 0.1 M de hidróxido de sodio se requiere para cambiar el color del indicador a rosa.
Rotación óptica especifica (2.2.7): + 54.4 a + 55.9 (sustancia anhidra).
Disolver 10.0 g en 80 ml de agua R, calentar hasta 50 °C. Dejar enfriar y añadir 0.2 ml de amoniaco diluido R1. Dejar en reposo por 30 min. y diluir a 100.0 ml con agua R.
Absorbencia (2.2.25).
Solución de prueba (a). Disolver 1.0 g en agua R hirviendo y diluir a 10.0 ml con el mismo solvente.
Solución de prueba (b). Diluir 1.0 ml de solución de prueba (a) a 10.0 ml con agua R.
Rango espectral: 400 nm para la solución de prueba (a) y 210-300 nm para solución de prueba (b).
Resultados:

Metales pesados (2.4.8): máximo 5 ppm.
Disolver 4.0 g en agua R con calor, añadir 1 ml de 0.1 M de ácido clorhídrico y diluir a 20 ml con agua R. 12 ml de la solución cumple con la prueba A. Preparar la solución de referencia usando solución estándar de plomo (1 ppm Pb) R.
Agua (2.5.12): 4.5 por ciento a 5.5 por ciento, determinado sobre 0.50 g, usando una mezcla de 1 volumen de formamida R y 2 volúmenes de metanol R como solvente.
