Discussion Olay yerinde en sık rastlanan biyolojik delil kan lekeleridir. Olayın oluş şekline göre bu lekelerin ne kadar süre beklediğini bazen bilemeyebiliriz. Olayın üstünden haftalar, aylar hatta yıllar geçebilmektedir. Ayrıca bu lekeler çeşitli yüzeyler üzerinde de bulunabilirler. Olay yerinde en çok karşılaşılan durumlar: Kanın; penye, havlu, koltuk döşemesi gibi yüzeylere bulaşmış olması, kapı, kalebodur, parke gibi yüzeylere sıçraması ya da akması ve en çok kullanılan cinayet aletlerinden biri olan bıçağın kabzasına sızmasıdır. Ancak olay yerinden gelen bu tür örnekler ya eser miktarda bulunmaktadır ya da bozulmuştur. Bu tür örneklerde DNA üzerinde daha çok yer kaplayan klasik STR lokusları ile tipleme sorunu yaşanabilmekte ve sonuç alınamadığı bilinmektedir. Bu çalışmada, belirlenen 6 mini STR lokusun bozulmuş örneklerde kullanılabilirliğini belirmek için temsili bir olay yeri oluşturuldu. Olay yerinde bulunabilecek çeşitli yüzeylere (kot, penye, döşeme, havlu, bıçak kabzası, laminant parke, kapı kilit demiri ve kalebodur arası derz dolgu) kan damlatılarak çeşitli sürelerde (1 hafta, 1 ay, 3 ay ve 6 ay) bekletildi. İzole edilen örneklerin miktar tayini Florometrik yöntemle yapıldı. Tablo 1 incelendiğinde bıçak ve demir gibi sert ve pürüzsüz yüzeylerden DNA diğer yüzeylere göre daha fazla miktarda geri kazanıldı. Yumuşak yüzeylerden en fazla DNA geri kazanımı penye, en az ise havluda görülmektedir. Havluda DNAnın geri kazanımının zor olmasının nedeni kumaş yapısının porlar içermesi ve yüzeyden uzayan iplikciklerin DNA’yı kumaş içinde hapsetmesi ve ekstraksiyon çözeltisine geçişini engellemesi olabilir. Bu nedenle ilk denemelerde havludan başarılı izolasyon yapılamadı. Ancak inkübasyon süresinin 1 saatten 24 saate çıkarılması ile bir miktar DNAnın ekstraksiyon çözeltisine geçişi sağlanabilmiştir. Pürüzsüz yüzeylerde ise DNA’nın ekstraksiyon çözeltisine geçişini engelleyen bir faktör bulunmadığı için daha başarılı sonuçlar alınmıştır. Elde edilen DNA miktarları zaman ve degredasyon yönünden değerlendirildiğinde ise zamana bağlı olarak DNA miktarındaki azalmanın tüm yüzeylerde hemen hemen aynı oranda olduğu görülmektedir. Genotip tayininde 1 hafta ile 3 aylık örneklerde hiçbir sorunla karşılaşılmadı. Ancak 6 ay bekletilmiş örneklerde PCR için yeterli miktarda DNA (0,1 ng/μl) bulunmasına rağmen tüm yüzeylerde amplifikasyon başarısız oldu. Bu sorun PCR döngü sayısının 32’den 34’e çıkarılması ile çözülmüştür. Elde edilen genotipler Tablo 4-7 de sunulmuştur. Bu tablolarda görüldüğü gibi tüm yüzeylerde ve sürelerde aynı kan kullanıldığı için aynı genotip elde edilmiştir. Bu sonuçların güvenilir olduğunu göstermektedir. Benzer şekilde çeşitli degrade niteliğindeki biyolojik örneklerle yapılmış Mini STR ile kimliklendirme çalışmaları mevcuttur (12). Ancak benzer çalışmada kullanılan mini STR setleri yeni mini STR (NC01-NC02) setlerini içermemektedir. Hazır ticari formattaki mini STR kitleri ile çalışma yapılmıştır ve yine alel düşmeleri gözlenmiştir. Dolayısıyla çeşitli degrade niteliğindeki biyolojik örneklerde yeni Mini STR seti NC01 ve NC02’nin kimliklendirme performansı saptanmalıdır. 5. Conclusion Bu çalışmada olay yerlerinde en fazla karşılaşılan çeşitli yüzeyler üzerinde 6 ay oda ısısında ve dış etkilere maruz koşullarda beklemiş kan lekelerinde miniSTR lokusları kullanılarak güvenilir bir şekilde genotip tayini yapılabileceği saptanmıştır. Bundan sonraki çalışmalarda süre uzatılarak bir ya da birkaç yıl bekletilmiş kan lekelerinde de çalışmanın tekrarlanması gereklidir. Ayrıca yöntemin degrade olmuş diğer biyolojik örneklerde (sperm, kıl, tükürük vb.) de kullanılabilirliği saptanması gereklidir ve öncü çalışma niteliğindedir. In calculation of the baseline DNA quantity considering the quantity obtained from the blood spot waited for 1 week, maximum DNA quantity was obtained from hosiery with 71.42%, while minimum recovery rate was obtained from towel with 16.66% and jeans with 28.57% at the end of 6 months. Recovery rate was 55% from furniture, glazed tile and laminated flooring and 67% from knife and iron. An some problems occurred during isolation of towel and incubation time was increased to 24 hours from 1 hour. The primary series and sizes of loci are given in Table 2. Table 2. Primary Series and Sizes of Loci A total of 25 μL of PCR mixture was prepared, which contains containing 1ng/ μL genomic DNA, 12,5 μL TopTaq™ Master Mix Kit - QIAGEN® and each primary set for each sample. Sensitivity study (K562) control (given as 994 A above) DNA sample was diluted to concentrations of 0.005, 0.025, 0.05, 0.1 and 0.5 ng/μl and minimum DNA ratio that can be used was determined for 6 miniSTR loci using such concentrations. It was observed that a peak can be achieved with DNA concentrations of 0.5 and 0.1 ng/μl DNA. No peak could be achieved with DNA concentrations of 0.05, 0.025 and 0.005 ng/μl (Table 8). Discussion The most commonly observed biological evidence in crime scenes is blood spots. Sometimes, we are unable to determine how long such blood spots waited depending on the occurrence of the event. Weeks, or even years, might have been passed since the occurrence of any event. In addition, such spots may be on various surfaces. The most commonly observed conditions in crime scenes include: Some surfaces, such as hosiery, towel, furniture, contaminated with blood; blood splashed or flowed on surfaces, such as doors, glazed tile, etc. and blood leaked on the grip of a knife, which is one of the most commonly used murder weapon. However, such samples are found in trace quantities or they have degraded in crime scenes. It is known that, in such samples, typology problems may occur with STR loci, which occupy a larger area on DNA, and no result can be achieved. In this study, a representative crime scene was created in order to determine of 6 mini STR loci determined in degraded samples. Blood was dropped on various surfaces (jeans, hosiery, furniture, towel, knife grip, laminated flooring, door lock bar and glazed tile grouting) in crime scene and waited for various durations (1 week, 1 month, 3 months and 6 months). Assay of the isolated samples was performed using Fluorometric methods. DNA recovery rate is higher on hard and smooth surfaces, such as knife and iron, according to Table 1. In case of soft surfaces, maximum DNA recovery is from hosiery and minimum DNA recovery is from towel. The difficulty of recovering DNA from towel is due to the pores in its structure and the filaments on the surface confining DNA within the fabric and preventing its passage to the extraction solution. Therefore, no successful isolation could be made from towel on the first tries. However, some passage of DNA into the extraction solution could be achieved by increasing the incubation time from 1 hour to 24 hours. In case of smooth surfaces, more successful results could be achieved, as there is no factor preventing passage of DNA into the extraction solution. When DNA quantities obtained were assessed in terms of time and degradation, it was observed that the decrease in DNA quantity was almost the same on all surfaces. No problem was observed in determination of genotype in samples waited for 1 week and 3 months. However, in samples waited for 6 months, amplification was unsuccessful on all surfaces, although there was a sufficient quantity of DNA (0,1 ng/μl) for PCR. This problem was resolved by increased PCR cycle number from 32 to 34. The genotypes obtained are given in Table 4-7. As seen on these tables, the same genotype was obtained as the same blood was used on all surfaces and times. This indicates that the results are reliable. Similarly, there are some studies on identification with Mini STR performed using biological samples at various degradation levels (12). However, mini STR sets used in a similar study do not include new mini STR (NC01-NC02) sets. A study was performed with commercially available ready-to-use mini STR kits and a decrease was observed in allele, as well. Therefore, identification performance of new Mini STR set NC01 and NC02 should be determined in the biological samples at various degradation levels. 5. Conclusion In this study, it was found that genotype can be determined reliably using miniSTR loci in blood spots on various surfaces, which are most common in crime scenes, exposed to external factors and waited at room temperature for 6 months. In the further studies, this waiting period should be extended and the study should be repeated on the blood spots waited for one or a few years. In addition, it should also be determined if this method can be used other biological samples (sperm, hair, saliva, etc.) and this study is a leading study.

Özet İntihar, kişinin üst bilincine (özbenliğine) yönelik bir saldırı olup, kişinin kendi hayatını istemli olarak sonlandırmasıdır. Tedavisi imkânsız hastalıkla yaşamak zorunda olan hastanın, kendi isteği doğrultusunda acı ve ağrısını sonlandırmak için tıbbi yoldan yaşamına son verilmesine ötanazi denir. Self ötanazi, intihar ile benzeşmesine rağmen, intiharda kişi kendi bildiği yöntem ve yollarla kendi isteğiyle hayatına son verirken, self ötanazi de ise ötanazi kararı ve iradesi hastadan gelir ancak ölüm sonucunu meydana getiren eylem tıp mensubu kişilerce yerine getirilir. Olgumuz terminal dönem akciğer kanseri teşhisiyle Göğüs Hastalıkları Hastanesi‘nde tedavi gören 60 yaşında erkektir. Kendisinin istemiyle oğlunun getirdiği silahla başından vurulmuş halde hastanede yatağında ölü bulunmuştur. Otopside sağ akciğerde yassı epitel hücreli karsinom tespit edilmiştir. Terminal dönem akciğer kanseri teşhisiyle hastanede hayatına son veren olgu eşliğinde intihar ve ötanazi kavramlarının ayrımı ve ötanazi hakkında kanuni sorunların tartışılması amaçlanmıştır. İntihar etmede erkeklerin kadınlara oranı 3-4/1 iken, intihar girişimlerinde bu oran tam tersi olmaktadır (3). Erkeklerin seçtiği intihar yöntemleri, kadınların seçtiği yöntemlere göre daha öldürücü özelliktedir. Erkekler yöntem olarak daha çok ateşli silahlar, ası gibi yöntemleri tercih ederken, kadınlar ise sıklıkla yüksek dozda ilaç alma, zehirlenme, bileklerini kesme gibi yöntemleri tercih etmektedirler (3,4). İntihar eden kişilerde özgüven yitimi ön plandadır (5). İntihar girişiminde bulunanların kişilik özellikleriyle ilgili yapılan araştırmalarda, bu kişilerin yaşam stresörleriyle karşılaştıklarında, kendilerine daha çok zarar verme eğiliminde oldukları gözlenmiştir (6). Geleceğe dair umutsuzluğun ve bedensel şikâyetlerin artması, ağrıya dayanma gücünün azalması gibi faktörler yaşlı kişilerde intihar krizini arttırmaktadır (7). Ötanazi; tedavisi imkansız hastalıkla yaşamak zorunda olan hastanın, kendi isteği doğrultusunda acı ve ağrısını sonlandırmak için tıbbi yoldan hastanın yaşamına son verilmesidir (8). Self ötanazi, kişinin kendi ölümüne, kendi isteğiyle razı olması durumudur. Ötanazi uygulama yönüyle ikiye ayrılır. Aktif ötanazide doktor, ani ölüm yapabilen öldürücü dozdaki ilacı uygulayarak hastasının hayatına son verir. Pasif ötanazi ise; doktor, hastasının bir süre daha yaşamasını sağlayacak tedaviyi vermez ya da tedaviyi sonlandırarak ölüm sürecini hızlandırır (9,10). Ülkelerin ötanaziye bakış açıları farklıdır. Bu bağlamda bakıldığında ötanaziyi yasal olarak kabul eden ilk ülke Hollanda’dır (11). Belçika’da ötanazi uygulaması 2002 yılında yasal hale gelmiştir (9, 11). İsviçre’de ötanazi, kanunlarla düzenlenmiş değildir ancak hastanın yaşamının sonlandırılmasına yardım eden kişilere yönelik bir yaptırım uygulanmamaktadır. İsviçre’deki uygulamada hastanın ölümüne yardım eden sadece doktor olmayıp, aynı zamanda başka kişiler de hastanın ölümünde yardımcı olabilmektedir (11). Türkiye’de, ötanazi konusunda Türk Ceza Kanunu (TCK)’nda özel bir hüküm bulunmamakta olup, Tıbbi Deontoloji Nizamnamesi’nin 14. Maddesinin 1. fıkrasında ve Sağlık Bakanlığı’nın Hasta Hakları Yönetmeliği’nin 13. maddesinde ötanazinin yasak olduğu belirtilmektedir (12). Dinler arasında da ötanaziye yaklaşımlar farklıdır. Tek Tanrılı dinler ötenaziyi cinayet olarak görüp reddeder. Doğu dinlerinden olan Şintoizm ve Budizm tedavisi imkânsız hastalık durumlarında ötanaziye izin vermektedir (11). Olgu Olgumuz terminal dönem akciğer kanseri teşhisiyle Göğüs Hastalıkları Hastanesinde tedavi gören 60 yaşında erkektir. Savcılık tutanaklarında; kanser tedavisi gören hastanın, oğlundan evden silah getirmesini istediği ve getirilen 9 mm çaplı silahla başından vurulmuş halde hasta yatağında ölü olarak bulunduğu kayıtlıdır. Ölen kişinin kardeşinin savcılıkta verdiği ifadede; ölen kişinin oğlunu ve kendisini görüşmeye çağırdığını, görüşmeden hemen sonra ölen kişinin oğlunun ‘’babam yalnız kalmak istiyor, dışarı çıkalım amca’’ diyerek elinden tutarak odanın dışına çıkardığını, koridorda aile bireylerinden diğer kişilerin de bulunduğunu, çok kısa bir süre sonra yeğeninin elleriyle kendi yüzünü kapattığını, yengesinin ’’ağlayabilirsin oğlum’’ demesi üzerine yeğeninin ağlayarak hastane koridorlarında koşmaya başladığını, bu sırada odadan silah sesi duyduğunu ifade etmiştir. Nöbetçi hemşirenin verdiği ifadede; bu olay cereyan etmeden önce ölen kişinin kendisinden helallik istediğini ifade etmiştir. (OLAY YERİ FOTO) Olgunun İstanbul Adli Tıp Kurumu Başkanlığı Morg İhtisas Dairesi'nde yapılan otopsisinde; dış muayenede sağ temporal orta bölgede etrafında stampa ve pim izi bulunan mermi çekirdeği giriş yarası, sol temporooksipitoparietal bölgede mermi çekirdeği çıkış yarası tespit edildi (Resim 1). Otopside sağ temporal orta bölgede 1 cm çaplı etraf kemik ve yumuşak dokuda is bulunan ateşli silah mermi çekirdeği giriş defekti olduğu görüldü (Resim 2). Beyin yüzey ve kesitlerinde mermi çekirdeği geçişine bağlı yaygın subaraknoidal kanama ile traje boyunca doku harabiyeti ve kontüzyonel değişimler olduğu görüldü. Sağ akciğerde ileri derecede plevral kalınlaşma, yapışıklık, sağ ana bronş ve bronşiollerde ileri derecede sertleşme ve çevre akciğer parankim dokusunda sınırları seçilemeyen sert bir kitle olduğu görüldü (Resim 3). Otopsi sırasında alınan doku örneklerinde yapılan toksikolojik incelemede kanda tedavi dozlarında Morfin ve Tramadol saptandı. Sağ ana bronş etrafındaki kitleden alınan örneğin yapılan histopatolojik incelemesinde sağ akciğerde yassı epitel hücreli karsinom tespit edildi (HİSTO PREPERAT) Kişinin ölüm sebebinin kafa bölgesine bitişik atış mesafesinden yapılan ateşli silah mermi çekirdeği yaralanması sonucu meydana gelmiş olduğu rapor edildi. Tartışma ve Sonuç Hollanda’da ötanazi isteğinde bulunanların %80 oranında kanser hastaları olduğu bildirilmiştir (9,10). Belçika’da 2004-2005 yıllarında ötenazi uygulanan 742 kişinin hastalıklarına göre dağılımı incelendiğinde, birinci sırada 618 hasta (%83) ile kanser hastaları, ikinci sırada ise nöromüsküler hastalığı olan kişiler yer almaktadır (13). Olgumuz intihar eylemini kanser tedavisi görmekte olduğu hastanede gerçekleştirmiştir. Ayrıca yapılan otopsi ve histopatolojik inceleme sonucunda olgumuzda sağ akciğerde yassı epitel hücreli karsinom tespit edilmiştir. Olgumuzun intihar eylemini kanser hastası olması nedeniyle yaşadığı sıkıntılar nedeniyle gerçekleştirdiği düşünülmüştür. Kanser teşhisi konulmuş olan hastalarda, hem intihar hem de intihar girişimi oldukça yüksektir (3). Bunun nedenlerinden biri de, kanserin şiddetli ağrıya neden olan hastalıkların başında gelmesidir (1). Olgumuzun kan örneklerinde kişinin şiddetli ağrı çekmesi sebebiyle hekimlerce verilmiş narkotik analjezikler(Morfin ve Tramadol) saptanmıştır. Self ötanazi, intihar ile benzeşmesine rağmen, intiharda kişi kendi bildiği yöntem ve yollarla kendi isteğiyle hayatına son verirken, self ötanazi de ise ötanazi kararı ve iradesi hastadan gelir ancak ölüm sonucunu meydana getiren eylem sağlık personelinin yardımı ile yerine getirilmektedir (9,10). Olgumuz intihar eylemini kendisi gerçekleştirmiştir. fakat hastanede bulunduğu için intihar eylemini gerçekleştirirken oğlundan yardım almak zorunda kalmış ve silahı kendisine oğlu getirerek bu kararını uygulamasında ona yardımcı olmuştur. 5237 sayılı Türk Ceza Kanununun 84/1 maddesinde; başkasını intihara azmettiren, teşvik eden, başkasının intihar kararını kuvvetlendiren ya da başkasının intiharına herhangi bir şekilde yardım eden kişiye iki yıldan beş yıla kadar, 84/2 maddesinde; intiharın gerçekleşmesi durumunda, kişiye dört yıldan on yıla kadar hapis cezası verileceği belirtilmektedir (10). Ceza kanunlarımızda ötanaziyle ilgili bir hüküm bulunmadığından, olgu olarak sunduğumuz bu tabloda ölen kişiye silah temin eden aile fertleri tespit edilerek bu kanun maddesi çerçevesinde yargılanması söz konusu olabilecektir. Kanunlarımızda ötanazinin uygulanması serbest bırakılmış olsaydı ölen kişi ötanazi hakkını kullandığı için kendisine yardım eden aile fertlerinin yasalar önünde suçlu sayılmayabileceği değerlendirilmiştir. 22 Mayıs 2014 tarihli 29007 sayılı Resmi Gazetede yayımlanan Sağlık Bakanlığı Yönetmeliğinde temel ilkeler bölümü Madde 5 1g fıkrasında “sağlık meslek mensupları ile sağlık hizmetlerinde çalışan diğer meslek mensupları hasta ve çalışan güvenliğini sağlamak amacıyla güvenli çalışma ortamının sağlanması ve sürdürülebilirliği ile muhtemel risklerin giderilmesine yönelik uygulamaları yapmak veya yapılmasını sağlamakla sorumlu olduğu” belirtilmektedir (14). Olgumuzda da görüldüğü gibi gerek hastanedeki sağlık çalışanları gerekse hastane güvenlik görevlileri olayda kullanılan silahın hastaya ulaşmasını engelleyememiş ve önemli bir güvenlik zafiyeti ortaya çıkmıştır. Dünyada ötanaziye yönelik standart bir bakış açısı bulunmadığından her ülkenin yasalarına göre değişen farklı yaklaşımlar söz konusudur (9). Türk Ceza Kanununda yer almayan ötanazi kavramı hukukçular, kanun koyucular ve sağlık çalışanları ile birlikte multidisipliner bir çalışma yapılarak bu konudaki yasal boşluğun giderilmesi gerektiğini düşünmekteyiz. Olgumuzda da görüldüğü gibi kanunlarımızda ötanaziyle ilgili düzenleme bulunmadığından kişi kendi yöntemiyle hayatına son vermiştir. Fakat hem oğlu hem de hastane çalışanları suçlu durumuna düşmüştür. Tüm bu yaklaşımların yanında koruyucu hekimlik bağlamında yaklaşıldığında kanser hastalarının gerek çektiği acılar, gerekse psikolojik yakınmaları göz önünde bulundurularak intihara meyilli olabilecekleri, bu nedenle kanser tedavisinin yanında muhakkak psikiyatrik tedavi de eklenerek destek olunmaya çalışılmalıdır.Olgumuzun kan örneklerinde yapılan kimyasal analizde herhangi bir psikiyatrik ilaç etken maddesi rastlanılmadığı dikkate alındığında, bu yönden hastaya yetersiz yaklaşımda bulunulduğunu düşünmekteyiz.	Translation - English The Case of Family-Assisted Termination of Life: Euthanasia or Suicide? Abstract Suicide is an attack at one's overconscious and a voluntary termination of one's own life. The practice of ending one's life through medical means in order to put an end to one's pain and suffering at one's own request when it comes to a patient with an incurable illness is called euthanasia. Even though suicide resembles self-euthanasia, it is the practice of ending one's own life through methods and means that he/she knows about whereas self-euthanasia includes a decision and a will of a patient for euthanasia. However, the action to result in death is taken by medical professionals. The case of this study is a 60-year old male who underwent a therapy at the Chest Disease Hospital due to a diagnosis of terminal lung cancer. He was found dead on his bed as he had been shot in the head by a gun provided by his son at his own request. The autopsy pointed to a squamous-cell carcinoma in the right lung. In line with a case of ending one's own life at a hospital due to a diagnosis of terminal lung cancer, this study aims at discussing over the distinction between suicide and euthanasia, and the legal issues about euthanasia. Keywords: euthanasia, suicide, death, autopsy Introduction Suicide is an attack at one's overconscious and a voluntary termination of one's own life (1,2). The ratio of men to women for suicide is 3-4/1 while it is quite the opposite when it comes to suicide attempts (3). The methods opted by men are more fatal when compared to the ones opted by women. Men usually opt for firearms and hanging whereas women typically go for overdose medication, poisoning or slitting wrists (3,4). The loss in self-confidence stands out in those who commit suicide. Studies on personalities of those who commit suicide point out that such people are more inclined to harm themselves when they face life stressors (6). Factors such as an increase in hopelessness for the future and in the number of physical complaints as well as a decrease in pain tolerability exacerbate suicide crises for elderly people (7). Euthanasia is the practice of ending one's life through medical means in order to put an end to one's pain and suffering at one's own request when it comes to a patient with an incurable illness (8). Self-euthanasia is a consent for one's own death at his/her own request. Euthanasia is divided into two in practice. For active euthanasia, a doctor ends a patient's life by administering a terminal dose of medication with an effect of sudden death. For passive euthanasia, a doctor accelerates the process of death by ceasing the therapy that would enable a patient to keep living, or by putting an end to the therapy (9,10). Countries offer various perspectives for euthanasia. The Netherlands is the very first country to legalize euthanasia (11). The practice of euthanasia became legal in Belgium in 2002 (9,11). In Switzerland, euthanasia is not regulated by law whereas there is no sanction for those who assist a patient to end his/her life. In Switzerland, it is not only doctors but also other people that are allowed to assist a patient to end his/her own life (11). In Turkey, there is no particular provision in the Turkish Penal Code for euthanasia while the paragraph 1 of the article 14 of the Medical Deontology Regulation, and the article 13 of the Ministry of Health's Regulation on Patient Relatives illegalize euthanasia (12). Religions offer various perspectives for euthanasia. Monotheistic religions regard euthanasia as a murder and reject the concept. Shintoism and Buddhism allow for euthanasia in case of incurable diseases (11). Case The case of this study is a 60-year old male who underwent a therapy at the Chest Disease Hospital due to a diagnosis of terminal lung cancer. Prosecution office's reports show that the patient administered with a cancer therapy asked his son to bring a gun from home and he was later found dead with a shot in the head with a 9 mm gun. According to the statement given by the deceased's brother, the deceased had called his son and him for a meeting, and after the meeting, the son took him outside as his father reportedly wanted to be alone. Other members of the family stood in the hospital corridor, and in a short while, his nephew covered his own face with his hands. After his aunt told him that he can cry, he started to run in the hospital corridors, and a gunshot was heard out of the room at the same time. The statement given by the on-call nurse points out that the deceased asked for his blessing before this incident took place. (SCENE PHOTO) Performed by the Morgue Specialization Department of the Istanbul Forensic Medicine Institute, the autopsy of the case revealed a wound caused by a bullet in the right temporal lobe with an imprint around, and in the left temporooccipitoparietal junction (Photo 1). The autopsy also pointed to an entrance defect of the firearm bullet with black smut in the soft tissue and 1 cm-diameter bone in the right temporal lobe (Photo 2). It was also clear to see extensive subarachnoidal bleeding induced by the bullet entrance on the brain surface and segments as well as tissue destruction and contusional changes throughout the trajectory. An advanced pleural thickening and cohesiveness in the right lung, an advanced hardening in the right main bronchus and bronchioles, and a rigid mass with no clear margin in the surrounding lung parenchymal tissue were detected, as well (Photo 3). Performed in line with tissue samples taken during the autopsy, the toxicological testing identified Morphine and Tramadol in blood in therapy doses. Performed in line with tissue samples taken from the mass surrounding the right main bronchus, the toxicological testing pointed to squamous cell carcinoma in the right lung (CYTOPREPARATION). The cause of death was reported to result from an injury caused by a firearm bullet shot at a distance adjacent to to the head. Discussion and Conclusion 80 % of those demanding euthanasia in the Netherlands reportedly consists of cancer patients (9,10). An analysis of diseases over 742 patients administered with euthanasia in 2004 and 2005 in Belgium set forth that cancer ranks first in this category with 618 patients (83%) and it is followed by neuromuscular diseases (13). The case of this study committed suicide at the time when he was undergoing a cancer therapy. In addition, it was deduced from the autopsy and the histopathological test that the case had squamous cell carcinoma in the right lung. It is considered that the case committed suicide due to his troubles resulting from the fact that he was a cancer patient. Both suicide and suicide attempts are extremely common for patients diagnosed with cancer (3). One of the reasons behind this fact is the severe pain caused by cancer (1). The blood samples of the case pointed to the administration of narcotic analgesics (Morphine and Tramadol) by doctors due to his severe pain. Even though suicide resembles self-euthanasia, it is the practice of ending one's own life through methods and means that he/she knows about whereas self-euthanasia includes a decision and a will of a patient for euthanasia. However, the action to result in death is taken by medical professionals (9,10). The case took the action of suicide on his own. However, he had to ask his son for help to commit suicide, and consequently his son assisted him in putting the decision into effect by bringing a gun to him. According to the article 84/1 of the Turkish Penal Code No. 5237, any person who solicits, encourages a person to commit suicide, or supports the decision of aperson for suicide or helps the suicide action in any manner whatsoever, is punished with imprisonment from two years to five years while the article 84/2 states that in case of commission of suicide, the person who is involved in such act is sentenced to imprisonment from four years to ten years (10). As the Turkish penal code does not include a provision with regard to euthanasia, the family members providing the deceased with a gun shall be identified and subjected to a trial in line with the article of this law. In case the penal code allowed for the practice of euthanasia, the family members of the deceased assisting him in exercising his right for euthanasia would not be found guilty. The paragraph 1 of the article 5 of the Ministry of Health's Regulation on fundamental principles published in the Official Gazette No. 29007 and dated May 22, 2014 states that health occupation members and other occupational members in the healthcare industry shall be responsible for putting practices into effect or having them put into effect to eliminate potential risks for ensuring a safe and sustainable working environment in order to provide patients and their relatives with security (14). As it was clear to see in the case, both the healthcare professionals and the security staff of the hospital could not prevent the delivery of the gun to the deceased, and thus a major security gap surfaced. Since there is no uniform perspective for euthanasia around the world, each country has its own point of view in terms of laws (9). We are of the opinion that the legal loophole for euthanasia that is not covered by the Turkish Penal Code needs to be eliminated as a result of a multi-disciplinary effort to be exerted by legal experts, lawmakers and healthcare professionals. As one could see in this case, the deceased ended his life through his own method since there is no provision for euthanasia in the Turkish law. However, both his son and the hospital professionals became guilty. From the perspective of preventive medicine, cancer patients need to be provided with psychiatric assistance as well given the fact that they are inclined to commit suicide due to pain and psychological complaints they suffer. We think that the patient was insufficiently dealt with in consideration of the fact that no psychiatric medication with active ingredients was found as a result of the chemical analysis performed on the blood samples of the case.

Source text - Turkish PAROTİSDE KİTLE İLE BAŞVURAN AC SQUAMOZ HÜCRELİ KARSİNOM OLGUSU ABSTRACT Background: Akciğer (AC) skuamöz hücreli karsinomun parotise metastazı çok nadir görülen bir durumdur. Case presentation: Olgumuzda 60 yaşında erkek hasta kulak alt ön kısmında şişlik şikayeti ile kliniğimize başvurdu. Yapılan değerlendirmede ileri evre AC kanseri olduğu tesbit edildi. Parotis kitlesine yapılan insizyonel biyopside skuamöz hücreli karsinom tanısı konuldu. Hastaya kemoterapi başlandı. Kemoterapi sonunda parotis metastazında tamama yakın regresyon izlenirken AC deki primer tümörde de hafif regresyon gözlendi. Conclusion: Parotisde kitle ile başvuran hastalarda nadir de olsa metastaz ayırıcı tanıda düşünülmelidir. Olgumuzda literatürde çok nadir görülen (ikinci vaka) AC squamoz hücreli karsinomun parotise metastazı sunuldu. Ayrıca uygulanan tedavi ile parotis metastazında tamama yakın regresyon izlendi ki bu durum ileri evre AC kanserlerinde nadir bir durumdur. BACKGROUND Akciğer (AC) kanserleri günümüzde ciddi morbidite ve mortalitelere neden olabilmektedir. ABD de hem erkeklerde hemde kadınlarda kanserle ilişkili ölümlerde akciğer kasneri ilk sırayı almaktadır (1). Her yıl 200 bin yeni tanı konmakta olup bunlardan 160 bini ölmektedir (1). AC kansernin parotis metastazı yapması oldukça nadir görülen olgu olup bu metastazlarında büyük çoğunluğu küçük hücreli tipte görülmektedir (2-10). Yaptığımız literatür değerlendirmesinde sadece bir vakada skuamöz cell karsinomun parotis metastazı yaptığı tesbit edilmiştir (11). Olgumuzda kliniğimize kulak alt-ön kısmında kitle şikayeti ile başvuran ve yapılan tetkikler sonucunda skuamöz hücreli AC kanseri metastazı tanısı konulan hasta literatür eşliğinde tartışılmıştır. CASE REPORT 60 yaşında beyaz erkek hasta kliniğimize sağ kulak alt-ön tarafında şişlik şikayeti ile başvurdu. Hastanın kitlesi 3 aydır mevcuttu. Ara ara kitleden pürülan akınılar geldiğini ve kullanılan antibiyoterapi ile bu akıntıların düzeldiğini belirtti. Ancak kitle gittikçe büyümekteydi. Kronik sistemik hastalık ve meslek hastalığı olmayan hastanın 35 yıldır 20 paket yıl sigara kullanımı mevcuttu. Hastanın alkol kullanımı ve madde alımı öyküsü yoktu. Hastanın yapılan klinik değerlendirilmesinde sağ parotis kuyruğunda 3x4 cm boyutlarında, cildi erode etmiş, alttaki dokuya fikse kitle mevcuttu (Figure 1). Hastanın fasial paralizisi yoktu. Sol taraflı parotis gland muayenesi doğaldı. Boyun lenfatik değerlendirilmesinde patolojik lenf nodu hipertrofisi mevcut değildi. Hastanın baş ve boyun muyanesinde şüpheli görünen cilt lezyonu ile karşılaşılmadı. Hastanın yapılan parotis USG sinde kitle formasyonu olmadığı ve granulatif lezyon görünümü olduğu için metastaz olabileceği düşünüldü. Kitleden insizyonel biyopsi planlandı. Hastaya preoperatif sürecte ameliyat hazırlığı için akciğer grafisi çekildi. AC grafisinde 7x7 cm lik kitle tesbit edilince toraks BT çekildi. Hastanın toraks BT sinde sağda karina düzeyinden başlayıp subkarinal ve sağ hiler bölgeden sağ ana bronşu ve sağ pulmoner arteri çevreleyerek üst lob anterior segment-orta lobda perifere doğru uzanan yaklaşık 7x7,5 cm boyutlarında yer yer nekrotik alanlar içeren yumuşak doku dansitesinde kitlesel lezyon tesbit edildi. Hastanın parotisinden alınan insizyonel biyopsi materyali patolojiye gönderildi. Patoloji sonucu squamoz hücreli karsinom metastazı olarak yorumlandı (Figure 2). Bunu takiben yapılan bronkoskopi, blagam sitolojisi ve transbronşial biyopsinin histolojisi tanıyı AC squamoz hücreli kanser olarak kesinleştirdi. AC tümörünün sistemik yayılımını değerlendirmek için hastaya PET-CT çekildi. FDG PET/CT scan with maximum intensity projection (MIP) (Figure 3A) and axial slices of PET (Figure 3B, 3E, 3H), CT (Figure 3C, 3F, 3I) and fusion (Figure 3D, 3G, 3J) images. PET/CT scan demonstrated hypermetabolic mass in the right parotis (arrow) and huge hypermetabolic cavitary lung mass in the right lung (arrow head) with hypermetabolic lymph nodes in the right hilum and subcarina. A hypermetabolic mass was seen in the right retroperitoneal region (thin arrow). Also, intense FDG uptake in the bilateral kidney parancyma were noted. Hasta onkoloji bölümüne yönlendirildi. Hastaya Gemsitabin ve Karboplatin kombinasyon kemoterapisi başlandı. 5 kür KT tamamlandıktan sonra kontrol amaçlı PET-CT çekildi. PET-CT de sağ parotis gland boyutlarında ve metabolik aktivitesinde büyük oranda regresyon (Figure 1) (Figure 4A, sağ akciğerde santral yerleşimli tümör boyutlarında hafifçe regresyon (Figure 4B) ve sağ retroperitoneal bölgede boyutlarında hafif progresyon izlendi (Figure 4C) DISCUSSION Parotis bezine metastazın primeri genellikle baş ve boyun bölgesindeki mukozal veya kutanöz tümörlerdir. Baş ve boyun bölgesi dışı metastaz kaynakları en sık akciğer, meme ve karaciğerdir (12). Parotis bezine diğer tükrük bezlerine göre daha yüksek oranda metastazın olmasının nedeni içerdiği lenf nodlarıdır. Parotisde ortalama 20-30 lenf folikülü ve lenf nodu bulunmakta olup lenfatik sistemle zengin bir bağlantısı mevcuttur (13). AC tümörlerinde hematolojik yayılım daha sık olsa da, literatürde parotis glandı parankimine metastaz gösterilmemiştir (14). Özellikle uzak tümörlerden parotise metastazın lenfatik yolla olduğu düşünülmektedir. Asemptomatik AC tümörlerinin parotise metastazı tümörün ilk klinik bulgusu olabilmektedir. Olgumuzda da hastanın akciğer ile ilgili klinik şikayeti mevcut değilken parotisde kitle oluşumu başlamıştı. Parotise metastaz durumunda tümörün özellikleri primer kaynağın özelliklerine benzer hızlı büyür ve fasial paraliziye neden olabilir (8). Olgumuzda da parotis metastazı 3 ayda 3x4 cm lik boyuta ulaşmıştı. Ancak fasial paralizi mevcut değildi. AC kanserlerinin subtipleri; squamous karsinom (%35), adenokarsinom (%30), küçük hücreli karsinom (%20) ve Large hücreli karsinom (%15) dir (10). Primeri AC olup parotise metastaz yapan olgu sayısı literatürde oldukça azdır (2-10). Bu olguların altı tanesi küçük hücreli kanser, 2 tanesi adenokarsinomdur. Sadece bir olguda primer tümör squamoz hücreli kanserdir. Olgumuz literatürdeki ikinci vakadır. Parotisdeki metastatik tümörlerin tedavisi tartışmalıdır. Baş boyun bölgesindeki tümörlerin parotise metastazında total/parsiyel parotitektomi ve RT önerilmektedir (15,16). Ancak uzak organlardan parotise metastaz olduğunda tedavi yöntemi değişmektedir. Bu durumlarda parotise yapılan işlemler daha çok tanıya yöneliktir. Çünkü AC tümörlerinin parotis metastazı ileri evre bulgusu olup prognozu kötüdür (8). Literatürdeki AC kanserlerinin parotise metastazında parotisdeki kitle için İİAB, insizyonel biyopsi, süperfisial parotitektomi, total parotitektomi şeklinde farklı yaklaşımlarda bulunulmuştur(2-10). Bizde olgumuzda tanı amaçlı parotisdeki kitleye insizyonel biyopsi uyguladık. Parotise metastaz yapan AC tümörlerinin tedavisinde kemoterapi ve/veya radyoterapi kullanılmaktadır. Borg’un bildirdiği squmoz cell AC karsinomunun parotise metastazı olgusunda RT kullanıldı (11). Hasta ileri evre olmasına rağmen RT sonrası 3. yılda yaşamına devam etmekteydi. Bizim olgumuzda ise hastaya 5 kür gemsitabin+ karboplatin kemoterapisi uygulandı. Parotis metastazında ciddi oranda regresyon izlendi. AC deki primer tümörde de regresyon izlendi. Ancak retroperitoneal metastazda hafif progresyon izlendi. Parotise metastaz yapan küçük hücreli AC kanserlerinde ise tedaviye rağmen hastaların büyük kısmı ilk yılı doldurmadan ex olmaktadır (2,3,8,9). Bu iki subtipin uzak organ metastazına verdiği sağkalım cevabı farklı olup bu konu ile ilgili ek çalışmalara ihtiyaç vardır. Parotisde kitle şikayeti ile gelen hastalarda nadir de olsa metastatik tümörler akla gelmelidir. Eğer bu tanı atlanır ise parotisdeki kitleye uygulanacak cerrahi hem fasial sinir hasarı oluşturabilecek hem de primer tümöre yönelik tedavide gecikmelere yol açabilir. CONCLUSION Parotisde kitle şikayeti ile gelen hastalarda nadirde olsa primerin AC olabileceği akılda tutulmalıdır. Olgumuzda parotise metastaz AC den squamoz hücreli kanser tarzında olmuştur. Bu literatürde ikini vakadır.Olgumuzda parotise metastazı tanısı konarak tedavi uygulanmış ve parotis metastazında ciddi oranda regresyon görülmüştür. Case presentation: A 60-years old male patient applied to our clinic with the complaint of mass at the lower frontal area of ear. As a result of an examination, it was determined that he had advanced lung cancer. In the incisional biopsy of the parotis mass, he was diagnosed to have squamous cell carcinom. Chemotherapy was started. While a regression was observed in metastasis to parotis at the end of chemotherapy, a slight regression was observed in the primary tumor in lung. Conclusion: A metastasis distinguisher diagnosis should be considered in case of patients applied with the complaint of parotis mass, even if it is a rare condition. In our case, metastasis of lung squamous cell carcinoma to parotis, which is a very rare condition (second case) in the literature, is presented. In addition, metastasis to parotis has regressed almost completely with the treatment administered and this is a rare situation in advanced lung cancer cases. BACKGROUND Today, lung cancer can cause severe morbidity and mortality. Lung cancer is at the top of the list of deaths associated with cancer in both males and females in USA (1). Every year, 200 thousand patients are diagnosed with lung cancer and 160 thousand of them die (1). Metastasis of lung cancer to parotis is a very rare condition and most of such metastasis are of small cell type (2-10). As a result of review of literature, only one case with metastasis of squamous cell carcinoma to parotis was determined (11). In our case, a patient, who applied us with the complaint of mass at the lower front area of the ear and was diagnosed to have metastasis of squamous cell lung cancer, was discussed in line with the literature. CASE REPORT 60-year old Caucasian male patient applied to our clinic with the complaint of mass at the lower front area of right ear. The patient had the mass for 3 months. He mentioned that purulent stream flowed out of the mass and this was recovered with anti-biotherapy. However, the mass was becoming larger. The patient, who had no chronic systemic disease and occupational disease, smoked 20 packages/year for 35 years. The patient had no alcohol and/or drug use. Following the clinical assessment of the patient, a mass of 3x4 cm fixed to the sub-tissue and eroded the skin was determined at the right parotis end (Figure 1). The patient had no facial paralysis. His left-hand parotis gland examination was natural. There was not pathological lymph node in the lymphatic assessment of the neck. No suspicious skin lesion was observed during examination of the head and neck of the patient. In the parotis USG applied to the patient, it was considered that there may metastasis, as there is no mass formation and appearance of granulative lesion. The incisional biopsy from the mass was planned. Lung graphy scan was performed during the preoperative period to prepare the patient for the operation. Upon determination of 7x7 cm mass in lung graphy, thorax CT scan was performed. In the thorax CT of the patient, massive lesions of about 7x7.5 cm with soft tissue density were determined, which start at the carina level at right and surround the right main bronchi and right pulmonary artery from the right hilar area and extend to the upper lobe anterior segment-medium lobe periphery and contain some necrotic areas. The incisional biopsy material taken from the parotis of the patient was sent to pathology lab. The result of pathology was interpreted as squamous cell carcinom metastasis (Figure 2). And the following bronchoscopy, pitutiary cytology and histology of transbroncihal biopsy confirmed the diagnosis of lung squamous cell cancer. PET-CT scan was performed in order to assess systemic spread of lung tumor. FDG PET/CT scan with maximum intensity projection (MIP) (Figure 3A) and axial slices of PET (Figure 3B, 3E, 3H), CT (Figure 3C, 3F, 3I) and fusion (Figure 3D, 3G, 3J) images. PET/CT scan demonstrated hypermetabolic mass in the right parotis (arrow) and huge hypermetabolic cavitary lung mass in the right lung (arrow head) with hypermetabolic lymph nodes in the right hilum and subcarina. A hypermetabolic mass was seen in the right retroperitoneal region (thin arrow). Also, intense FDG uptake in the bilateral kidney parancyma were noted. The patient was referred to oncology department. A combination chemotherapy with Gemcitabine and Carboplatin was started. After completing 5 cures, a PET-CT scan was performed for control purposes. In PET-CT scan, a significant regression in the size of right parotis gland and metabolic activity (Figure 1) (Figure 4A), a slight regression in the size of the right lung centrally located tumor (Figure 4B) and a slight progression in the size of the right retroperitoneal area (Figure 4C) were observed. DISCUSSION Metastasis in the parotis gland is primarily caused by mucosal and cutaneous tumors in the head and neck area. The sources of metastasis outside head and neck area are mostly lungs, breasts and liver (12). High rate of metastasis in the parotis gland than the other salivary glands is the lymph nodes it contains. There is an average of 20-30 lymph follicles and lymph in the parotis and it has a significant connection with the lymphatic system (13). Although the hematological spread in lung tumors is more common, no metastasis to parotis gland parenchyma was shown in the literature (14). Particularly, it is considered that metastasis from the distant tumors to parotis occurs via lymphatic route. Metastasis of asymptomatic lung tumors to parotis can be the first clinical funding of the tumor. In our case, there was formation of mass in parotis, while the patient had no clinical complaint about his lungs. In case of metastasis to parotis, the characteristics of the tumor are similar to those of the primary source and it rapidly grows and may cause facial paralysis (8). In our case, parotis metastasis had reached to 3x4 cm in 3 months. However, there was no facial paralysis. Subtypes of lung cancers include squamous carcinom (35%), adenocarcinoma (30%), small cell carcinoma (20%) and Large cell carcinoma (15%) (10). The number of cases with metastasis to parotis from lungs is very low (2-10). Six of such cases are small cell cancer and 2 of them are adenocarcinoma. Only one case is primary tumor squamous cell cancer. Our case is the second case in the literature. Treatment of metastatic tumors in parotis should be discussed. Total/partial parotidectomy and RT are recommended in case of metastasis of tumors in head and neck area to parotis (15,16). However, in case of metastasis from distant organs to parotis, the treatment method differs. In such cases, the operations on the parotis are rather aimed at diagnosis. Because metastasis of lung tumors to parotis is an advanced stage finding and its prognosis is bad (8). In metastasis of lung cancers to parotis in the literature, different approaches, such as İİAB, incisional biopsy, superfacial parotidectomy, and total parotidectomy, were used for the mass in parotis (2-10). And we applied incisional biopsy to the mass in parotis for diagnostic purposes. For treatment of lung tumors with metastasis to parotis, chemotherapy and/or radiotherapy is used. RT was used in case of metastasis of squamous cell lung carcinoma to parotis reported by Borg (11). The patient survived in year 3 following RT, even if his cancer was at an advanced stage. And in our case, 5 cures of gemcitabin + carboplatin chemotherapy were administered to the patient. A significant regression was observed in metastasis to parotis. A regression was observed in the primary tumor in lung. But, a slight progression was observed in the retroperitoneal metastasis. And in case of small cell lung cancer with metastasis to parotis, most of the patients die during the first year in spite of the treatment (2,3,8,9). The survival respond of these two subtypes to distal organ metastasis is different and additional studies are required. For patients, who applied with the complaint of mass in parotis, metastatic tumors should be considered, even if they are observed rarely. If this diagnosis is skipped, an operation in the mass in parotis will not only cause a facial nerve damage but also cause delays in treatment of the primary tumor. CONCLUSION For patients, who applied with the complaint of mass in parotis, it should be considered that the primary source can be lungs, even if it is a rare condition. In our case, metastasis to parotis is from squamous cell cancer in lung. This is the second case in the literature. In our case, the patient was treated with diagnosis of metastasis to parotis and a significant regression was observed in the metastasis to parotis. Background: KOAH’lı hastalarda solunum fonksiyonları ve gaz değişimi bozulduğundan, inhalasyon anestezik ajanların solunum yoluyla atılımı etkilenebilmektedir. Çalışmamızda sevofloran washout zamanı ile kronik obstrüktif akciğer hastalığı (KOAH) arasında ilişki olup olmadığını saptamayı amaçladık Method: Çalışmamıza 18-60 yaş arası, ASA 1-3, (KOAH) tanısı olan, operasyon için genel anestezi planlanan 60 hasta dahil edildi. Grup N (KOAH olmayan=33), grup KOAH: (KOAH ‘lı hastalar, n=33, 2 hasta çalışma dışı, toplam 31 hasta) olarak 2 gruba ayrıldı. Tüm hastalara; preoperatif solunum fonksiyon testi (SFT) yapılarak kaydedildi. Operasyon odasında genel anestezi için standart monitorizasyon sağlandı. Her iki gruba da genel anestezi indüksiyonu için propofol 2mg/kg, fentanyl 1,5-2 mcg/kg, rokuronyum 0,6 mg/kg intravenöz uygulandı, taze gaz akımı 6 lt/dk dan açıldı.İdamede oksijen-hava karışımı %50, tidal volüm 6 ml/kg (ideal vücut ağırlığı), sevoflurane 1 MAC olacak şekilde uygulandı, remifentanil 0,05-0,1 mcg/kg/dk iv infüzyon verildi. End-tidal CO2:35-40 mmHg, SpO2: %95 ve üzeri olacak şekilde anestezi cihazı ayarlandı. Hasta ekstübasyona kadar mekanik ventilatörde takip edildi, operasyon sonunda sevofloran vaporizatörü kapatılarak ölçüm süresi başlatıldı. Remifentanil IV infüzyonu, sevofluran 0,1 Minimum Alveoler Konsantrasyona(MAC) ulaşınca kesildi. Kas gevşetici ajanlar operasyon bitiminden 30 dakika önce kesildi. Operasyon boyunca FiO2, Fins(Sevo), Fexp(Sevo), End-tidal CO2 ve vaporizatör kapatılmadan önce de Fins(Sevo)/ Fexp(Sevo) oranı, Mac1,MAC2, MAC3,MAC4 ve ekstübasyon süreleri kaydedildi. Results: KOAH olan ve olmayan bireylerin solunum fonksiyon testleri ile MAC1, MAC2, MAC3, MAC4 ve extubation time arasında anlamlı düzeyde bir ilişki bulunmamıştır. (p>0.05). Yaşa göre düzeltme yapılarak, ölçülen MAC1, MAC2, MAC3, MAC4 ve ekstübasyon süreleri, fins1, fins2, fexp1, fins1/fexp1 oranı ve fins2/fexp2 oranı değerlerinin gruplar arasında anlamlı düzeyde farklı olmadıkları saptanmıştır. Conclusion: Her iki gruptaki wash-out ve ekstübasyon sürelerinde fark olmamasına rağmen belirgin havayolu obstrüksiyonunun yarattığı hava tuzağının, artmış BMİ ve intraoperatif hemoglobin değerlerindeki düşüşlerin KOAH grubunda dikkatle değerlendirilmesi gerektiğini düşünmekteyiz INTRODUCTION: Kronik obstrüktif akciğer hastalığı (KOAH) toplumda oldukça sık görülen bir patolojidir. KOAH tanısı medikal öykü, fizik muayene, radyolojik tetkikler önderliğinde konulmakla birlikte, bu teşhisin spirometre ölçüm değerleriyle de doğrulanması gerekmektedir(1,2). Havayolu obstrüksiyonu ve KOAH tanısı GOLD kriterlerine göre bronkodilatatör ajan uygulanması sonrası spirometri ile ölçülen FEV1/FVC oranının %70 altında olması olarak tanımlanabilir (2,3,4,5) Ayrıca bu oran ATS-ERS ve NICE guidelinelarına göre yaştan bağımsız olarak KOAH teşhisinde bir kriter olarak belirtilmiştir(5). KOAH tanısı medikal öykü, fizik muayene, radyolojik tetkikler önderliğinde konulmakla birlikte, bu teşhisin spirometre ölçüm değerleriyle de doğrulanması gerekmektedir(6) FEV1/FVC oranı akciğer fonksiyonlarının pratik bir göstergesidir, kolayca ölçülür, referans eşdeğerliğine gerek göstermez ve 60 yaş üzerinde belirgin olarak azalır(6). İnhalasyon anestetik ajanların eliminasyonları solunum yollarındaki gaz değişim yüzeyinin genişliği ve kalitesini bozan patalojilerden etkilenmektedir. Akciğerlerin uyandırma sırasında %100 oksijen ile yıkanması alveol içerisindeki inhalasyonel anestetik gazların konsantrasyonunu ve parsiyel basınçlarını düşürerek dokulardan alveollerin içine doğru difüzyonlarının artışına yol açarak uyanmanın sağlanması şeklinde olmaktadır. Sevofluran tüm dünyada oldukça yaygın kullanılan, güvenli bir volatil anestezikdir (2,7). Akciğerlerin difüzyon kapasitesi(DL) oksijenin alveollerin içerisindeki gaz karışımından eritrositler içine taşınmasının hesaplamasıdır. Oksijen transportu üç etkenden etkilenir. Bunlar alveolar kapiller membranın yüzey alanı(A), membranın kalınlığı(T) ve alveolar oksijen gaz basıncı ile venöz kandaki oksijen basınç (∆PO2) farkından ortaya çıkan itici basınçtır. KOAH’da havayolu tıkanıklığı büyük oranda akciğer dokusundaki inflamasyon, fibrosis ve distal havayollarındaki mukus tıkaçları nedeniyle alveolar duvarların bozulması ile karakterize amfizem nedeniyle olmaktadır.(8) İnhalasyon ajanlarının akciğerlerden geç atılımının derlenmenin kalitesini de etkileyebileceğinden yola çıkarak çalışmamızda sevofloran wash-out ve ekstübasyon zamanlarının KOAH olan ve olmayan hastalarda karşılaştırılmasını amaçladık. METHODS Düzce Üniversitesi invaziv olmayan araştırmalar etik kurul onayı alındıktan sonra ( Date:13/09/2012, Decision Nr:2012/302)( ClinicalTrials.gov Identifier: NCT02209883) Çalışmamıza 18-60 yaş arası, ASA (American Society Of Anesthesiologists Classification) Status 1-3, KOAH tanısı olan ve olmayan, operasyon için genel anestezi planlanan toplam 66 hasta dahil edildi. Çalışmada kullanılan ilaçlara allerjisi olan, son 2 senedir sigara kullanan, klinik olarak belirgin kardiyovasküler endokrin, nörolojik, metabolik hepatorenal hastalığı olan,restriktif tipte pulmoner bozukluğu; geçirilmiş akciğer operasyonu, gastroözefajial reflü, anamnezi olan obez (body mass index 30 üzerinde), hepatik, renal yetmezliği, pulmoner hipertansiyonu olan ve gebe hastalar çalışmaya kabul edilmedi. Çalışmaya alınan hastalar Grup N ( Kontrol grubu hastalar, n=33), Grup COPD (KOAH ‘lı hastalar, n=33) olarak 2 gruba ayrıldı. Grup COPD ‘dan 2 hasta intraoperatif hemodinamik instabilizasyon nedeni ile çalışma dışı bırakıldı. Tüm hastalara; preoperatif solunum fonksiyon testi (SFT) yapılarak değerler kaydedildi. Operasyon odasında sistolik, diyastolik ve ortalama kan basıncı, kalp tepe atımı, 3 lead’li EKG, oksijen saturasyonunu (SpO2) içeren standart monitorizasyon yapıldı. Her iki gruba da genel anestezi indüksiyonu için propofol 2mg/kg, fentanyl 1,5-2 mcg/kg, roküronyum 0,6 mg/kg intravenöz (IV) yoldan uygulandı. Tüm hastalar aynı tip anestezi cihazına bağlanarak ( Datex Ohmeda S/5 Avance, Datex Ohmeda Inc.USA) taze gaz akımı 6 lt/dk’ dan açıldı. İdamede oksijen-hava karışımı %50, tidal volüm 6 ml/kg (ideal vücut ağırlığı), sevoflurane 1 MAC düzeyinde kalacak şekilde uygulandı, Remifentanil 0,05-0,1 mcg/kg/dk iv infüzyon verildi. Operasyon boyunca tidal volüm:6-8 ml/kg, frekans: 10- 12 soluk/dakika, End-tidal CO2:35-40 mmHg, SpO2: %95 ve üzeri olacak şekilde anestezi cihazı ayarlandı. PEEP uygulanmadı. Hasta ekstübasyona kadar mekanik ventilatörde takip edildi, operasyon sonunda sevofloran vaporizatörü kapatılarak ölçüm süresi başlatıldı. Remifentanil IV infüzyonu sevofluran 0,1 Minimum Alveoler Konsantrasyona(MAC) ulaşınca kesildi. Kas gevşetici ajanlar operasyon bitiminden 30 dakika önce kesildi ve sevofloran kesilmeden önce TOF yanıtında 2 twich oluştuğu gözlenerek hastaya nöromuskuler bloğu geri döndürmek amacıyla neostigmin ( 1,5mg) ve atropin (0,5mg) iv uygulandı. ABSTRACT Background: As the respiratory functions and gas exchange deteriorate in patients with COPD, excretion of inhalational anesthetic agents via respiration can be affected. In our study, we aimed at investigating if there is any relationship between the washout time of sevoflorane and chronic obstructive pulmonary disease (COPD). Method: 60 patients diagnosed with ASA 1-3 COPD, who were aged between 18 and 60 years old and scheduled to have general anesthesia for operation, enrolled in our study. Patients were divided into 2 groups: Group N (non-COPD=33), group COPD: (patients with COPD, n=33, 2 patients were excluded from the study, a total of 31 patients). All patients underwent pre-operative respiratory function test (RFT) and their results were recorded. Standard monitoring was provided in the operation room. Both groups received propofol 2mg/kg, fentanyl 1.5-2 mcg/kg, rocuronium 0.6 mg/kg via intravenous route and fresh blood flow was started from 6 l/min. During maintenance, 50% oxygen-air mixture with tidal volume of 6 ml/kg (ideal body weight) and sevoflurane of 1 MAC, remifentanil was administered at 0.05-0.1 mcg/kg/min via iv infusion. Anesthesia device was set for End-tidal CO2:35-40 mmHg, SpO2: 95% and more. The patient was monitored with mechanical ventilator until extubation, sevoflorane vaporizer was closed at the end of the operation and the measurement time was started. Infusion of remifentanil IV was stopped when sevoflorane reached to Minimum Alveolar Concentration (MAC) of 0.1. Administration of muscle relaxants was discontinued 30 minutes before end of operation. FiO2, Fins(Sevo), Fexp(Sevo), End-tidal CO2 were recorded during the operation and Fins(Sevo)/ Fexp(Sevo) ratio, Mac1,MAC2, MAC3,MAC4 and extubation times were recorded before closing the vaporizer. Results: There was no significant relationship between the respiratory function tests of the individuals with or without COPD and MAC1, MAC2, MAC3, MAC4 and extubation time (p>0.05). It was determined that there was no significant difference between the groups in terms of measured MAC1, MAC2, MAC3, MAC4 and extubation times, fins1, fins2, fexp1, fins1/fexp1 ratio and fins2/fexp2 ratio, corrected for age. Conclusion: Although there was no difference between the washout and extubation times of both groups, the aerial trap caused by significant obstruction of the airway, increased BMI and decreased intraoperative hemoglobin values should be considered with due care in COPD group. In the COPD patient group, scheduled for operation under general anesthesia, the preoperative preparations should be aimed at correction of the related parameters. We believe that wider and more comprehensive studies should be performed in order to put forth the factors that affect excretion. Key Words: Sevoflurane, COPD, washout time, gas exchange, anesthesia INTRODUCTION: Chronic obstructive pulmonary disease (COPD) is a very common pathology. COPD is diagnosed based on medical history, physical examination, radiological examinations and should be verified using values measured by spirometer (1,2). Diagnosis of airway obstruction and COPD can be defined as FEV1/FVC ratio measured with spirometer after administration of bronchodilator agent below 70% according to GOLD criteria (2,3,4,5) In addition, this ratio is also defined as the criterion for diagnosis of COPD independently from age according to ATS-ERS and NICE guidelines (5). COPD is diagnosed based on medical history, physical examination, radiological examinations and should be verified using values measured by spirometer (6). FEV1/FVC ratio is a practical indicator of pulmonary functions, it can easily be measured, does not require reference equivalence and significantly decreases over 60 years old (6). Elimination of the inhalational anesthetic agents is affected by the pathologies that disturb width and quality of the gas exchange surface in the airways. Washing lungs with 100% oxygen during wake up period decreases the concentration and partial pressure of inhalational anesthetic gasses in the alveoli to cause their increased diffusion into the alveoli through the tissues and thus causes wake up. Sevoflurane is a safe volatile anesthetic, which is commonly used throughout the world (2,7). Diffusion capacity (DL) of the lungs is calculated based on the amount of oxygen transported from the gas mixture in the alveoli into the erythrocytes. Oxygen transportation is affected by three factors. These include the surface area of the alveolar capillary membrane (A), thickness of the membrane (T) and the repellent pressure caused by the difference of pressure between the alveolar oxygen gas pressure and oxygen in venous blood (∆PO2). The airway obstruction in case of COPD is mostly caused by emphysema characterized by the disruption of the alveolar walls due to the mucus traps in the distal airways, inflammation in the lung tissue and fibrosis.(8) Departing from the fact that the delayed excretion of the inhalation agents from the lungs may affect the quality of recovery, this study aims at comparing the washout and extubation times of sevoflorane between patients with and without COPD. Abbreviations: MAC: Minimum Alveolar Concentration FEV1: Forced Expiratory Volume in 1 second FVC: Forced vital capacity FEF 25-75 : Forced Expiratory Flow at 25-75% Hb: Hemoglobin Htc: Hematocrit COPD: Chronic Obstructive Pulmonary Disease ASA: American Society Of Anesthesiologists Classification BMI: Body Mass Index SpO2: Peripheric Oxygen Saturation METHODS Upon approval of the noninvasive researches ethical committee of Düzce University (Date:13/09/2012, Decision Nr:2012/302) (ClinicalTrials.gov Identifier: NCT02209883) 66 patients diagnosed with or without ASA (American Society Of Anesthesiologists Classification) Status 1-3 COPD, who were aged between 18 and 60 years old and scheduled to have general anesthesia for operation, enrolled in our study. The subjects, who have allergy to the drugs used in the study, are smokers for the last 2 years, have the clinically significant cardiovascular endocrine, neurological, metabolic hepatorenal disease, restrictive-type pulmonary disorder, underwent lungs operation previously, has gastroesophageal reflux, anamnesis, obese (body mass index over 30), hepatic, renal failure, pulmonary hypertension and are pregnant. The patients included in the study were grouped in 2 groups: Group N (Control group patients, n=33), Group COPD (patients with COPD, n=33). 2 patients in Group COPD were excluded from the study due to intraoperative hemodynamic instabilization. All patients underwent pre-operative respiratory function test (RFT) and their results were recorded. A standard monitoring was performed in the operation room, which included systolic, diastolic and mean blood pressure, peak pulse, 3-lead ECG, oxygen saturation (SpO2). Both groups received propofol 2mg/kg, fentanyl 1.5-2 mcg/kg, rocuronium 0.6 mg/kg via intravenous (IV) route for general anesthesia induction. All patients was connected to the same type of anesthesia device (Datex Ohmeda S/5 Avance, Datex Ohmeda Inc.USA) and the fresh blood flow was started from 6 l/min. During maintenance, 50% oxygen-air mixture with tidal volume of 6 ml/kg (ideal body weight) and sevoflurane of 1 MAC, remifentanil was administered at 0.05-0.1 mcg/kg/min via iv infusion. The anesthesia device was set for tidal volume:6-8 ml/kg, frequency: 10-12 respiration/minute, End-tidal CO2:35-40 mmHg, SpO2: 95% and more. No PEEP was applied. The patient was monitored with mechanical ventilator until extubation, sevoflorane vaporizer was closed at the end of the operation and the measurement time was started. Infusion of remifentanil IV was stopped when sevoflorane reached to Minimum Alveolar Concentration (MAC) of 0.1. Administration of muscle relaxants was discontinued 30 minutes before end of operation and neostigmin (1.5 mg) and atropine (0.5 mg) iv was administered to the patient to restore neuromuscular block after observing 2 twitches in TOF response before discontinuing sevoflorane. Olgumuz 87 yaşında olup Nisan 2014 te ölüm sebebinin araştırılması için savcılık tarafından İstanbul Adli Tıp Kurumu Başkanlığı Morg İhtisas Dairesi’ne gönderildi. Otopsisinde rastlantısal olarak intra-abdominal yerleşimli 12,5x8x5 cm ebadında, 227 gr ağırlığında femur ve humerus ölçümlerine göre 24-25 haftalık kalsifiye ölü bir fetus ile sağ overde 12 cm çapında kalsifiye kist tespit ettik. Ölenin sadece bir evlilik yaptığı, eşinin 1990 yılında öldüğü, en son 1946 yılında olmak üzere 3 kez doğum yaptığı bilgisine ulaştık. Olgumuzun menapoz yaşı bilinmediğinden bilinen son gebelik tarihi ile eşinin ölüm tarihi göz önüne alındığında lithopedion retention süresi 24-68 yıl olarak hesaplandı. Olgumuz Türkiye’de tanımlanan ve otopside rastlantısal saptanan ilk vaka olup literatürde tanımlanan en yaşlı olgulardan biridir. INTRODUCTION: Yunanca da “taş bebek” anlamına gelen Lithopedion, ilerlemiş bir ektopik gebelikte ölü fetusun kalsifikasyonu ile ilişkili oldukça nadir görülen bir durumdur. Lithopedion, primer abdominal gebelikten, uterus rüptürü ile sonuçlanan intrauterin gebelikten ya da tubal veya ovaryan ektopik gebelik rüptürü sonucu oluşabilir (1). Membranların ve fetusun kalsifikasyonuna göre 3 alt tipte sınıflandırılabilir; sadece membranların kalsifiye olduğu lithokelyphos, hem membranların hem de fetusun kalsifiye olduğu lithokelyphopedion, sadece fetusun kalsifiye olduğu lithopedion (2,3). Tipik bir semptomu olmamakla birlikte tanı koydurucu herhangi bir laboratuar testi de yoktur. Olgular sıklıkla başka sebeplerle çekilen batın grafilerinden ya da elle yapılan batın ve pelvis muayenesi sırasında hissedilen kitleden farkedilebilirler. Asemptomatik geçen uzun bir sürenin ardından pelvik ağrı ve batında ağırlık hissi ile kendini gösterebilir. Literatürde akut bağırsak ve üriner sistem tıkanmalarına yol açabildiği ve hatta malignite (extrauterine placental site trophoblastic tumour) ile ilişkisi bildirildiğinden tanısı konulduktan sonra en kısa zamanda cerrahi yolla çıkartılması önerilmektedir (1,4-6). CASE REPORT: Olgumuz Mart 1927 doğumlu olup Nisan 2014 yılında evinde öldüğü bildirilen 87 yaşında bir kadındır. Kızından alınan bilgiye göre ölenin akli dengesi yerinde olmayan oğlu ile birlikte yaşadığı, çok erken yaşta olmak üzere sadece bir evlilik yaptığı, 1944 yılı öncesinde, 1944 yılında ve en son 1946 yılında olmak üzere 3 kez doğum yaptığı ve eşinin 1990 yılında öldüğü bilgisine ulaştık. Bunun dışında ölenin düşük ile sonuçlanan gebelik yaşayıp yaşamadığı, gebelikten korunma yöntemi kullanıp kullanmadığı ve menapoz tarihi bilgisine ulaşamadık. Olgumuz evinde şüpheli bir durum olmamasına rağmen ölü bulunmuş olması ve bilinen bir rahatsızlığı olmaması nedeniyle ölüm sebebinin tespiti açısından otopsi yapılmak üzere savcılık tarafından İstanbul Adli Tıp Kurumu Başkanlığı Morg İhtisas Dairesi’ne gönderilmiştir. Otopsisinde, dış muayenede kişisel bakımının ve hijyeninin kötü olduğu, zayıf yapılı olduğu, her iki dirsekte ve ayak sırtında lokal sıyrıklar ve ekimozlar olduğu görüldü. Radyolojik görüntülemede batın sağ alt yanda kalsifiye fetüs imajı ve pelviste kalsifiye yuvarlak şekilli kistik membran izlendi (Fig.1). Batın sağ alt kadranda omentum majus hemen altına alt ekstremiteleri seviyesinde yapışık halde taşlaşmış fetus (lithopedion) izlendi (Fig.2). Plasenta ve diğer gebelik ekleri izlenmedi. Figure 2: intra-abdominal lithopedion Kalsifiye fetus 12,5x8x5 cm boyutunda olup femur uzunluğunu 4,9 cm, humerus uzunluğunu 4,6 cm ve ağırlığını 227 g ölçtük (Fig.3). Figure 2: Lithopedion aspects Uterus yüksekliğini 10,5 cm, uterus genişliğini 10 cm, myometrium kalınlğını 2 cm, sol overi 5,5x5x4,5 cm, sağ overi 5,5x4x4,5 cm ölçtük. Ayrıca sağ overde 12 cm çapında kalsifiye kist tespit ettik. Uterus ve tubalar sağlam olup rüptür bulgusu saptamadık. Duodenumda pilorun 1 cm distalinde etrafı kanamalı 0,5 cm'lik peptic ulcus perforasyonu tespit ettik. Kalpte kronik kalp yetmezliği ile birlikte akut myokard enfarktüsü lehine bulgular saptadık. Diğer göğüs ve batın organları ile beyinde geriatrik yaş ile uyumlu bulgular dışında özellik saptamadık. Kimyasal incelemede terapötik düzeyde Naproxen saptandı. Histopatolojik incelemede taze myokard enfarktüsü (ilk 24 saati ile uyumlu), myokardial nedbe alanı, akciğerde bazı orta ve büyük çaplı damar lümenlerinde fibrin trombüs, duodenum mukozasında otoliz ve eroziv değişiklikler, serozada ve omentumda peritonit bulguları tespit ettik. Sonuç olarak; Kişinin ölüm sebebini duodenal peptic ulcus perforasyonu ve buna bağlı gelişen komplikasyonlar (peritonit, akut myokard infarktüsü) sonucu meydana geldiğini rapor ettik. DISCUSSION: Lithopedion, intra-abdominal kalsifiye ölü fetüs olarak tanımlanır. Primer intra-abdominal gebelikten yada intrauterin veya tubal yerleşimli olup rüptür sonucu sekonder abdominal gebelikten gelişebilir (7). Lithopedion gelişimi için ekstra uterin yerleşimli fetüs en az 3 ay boyunca hayatta ve steril kalmalı, tıbben tanı konmamış veya konamamış olmalı ve kalsiyum çökelmesi için uygun şartların oluşması gereklidir (2). Literatürde yaklaşık 330 kadar taş bebek olgusu bildirilmektedir (8). Ektopik gebeliklerin % 1,5-2’sinde, tüm gebeliklerin %0,0054’ünde görülebilen ve sıklıkla rastlantısal saptanan nadir bir fenomendir (3). Sıklıkla düşük sosyoekonomik düzeydeki doğum öncesi bakımı olmayan kişilerde görülmektedir (1,2). Olgumuz 3 çocuk sahibi olup akli dengesi yerinde olmayan oğlu ile birlikte yaşamaktadır. Kızından alınan bilgiler her ne kadar tatmin edici değilse de ölenin öz bakımının kötü olması ve akli dengesi yerinde olmaya oğlu ile birlikte yaşıyor olması nedeniyle olgumuzun yaşam şartlarının iyi olmadığını düşünmekteyiz. Literatürde olguların yaşları 20 ila 100 aralığında değişkenlik gösterirken bu olguların 2/3’ü 40 yaşının üzerindedir. Lithopedion retention süresi 4 ila 65 yıl arasında değişir (1,7,8). Olgumuz literatürde tanımlanan en yaşlı (87 years old) olgulardan biri olup evinde ölü bulunmuştur. Olgumuzun menapoz yaşı bilinmediğinden, en son 1946 yılında doğum yaptığı ve eşinin 1990 yılında öldüğü göz önüne alındığında lithopedion retention süresi minumum 24 yıl, maksimum 68 yıl olarak hesaplanabilir. Fetal ölüm, olguların %20’sinde gebeliğin 3 ila 6 ayları arasında, %27’sinde 7 ila 8 aylar arasında ve %43’ünde ise full term de gerçekleşir (1). Olgumuzdaki lithopedion 12,5x8x5 cm ebadında, 227 gr ağırlığında olup fetal ölüm, femur ve humerus ölçümlerine (9) göre 25-29 hafta ile uyumlu bulundu. Tien 114 lithopedion olgusu toplamış ve bunlardan 74’ünün tubal gebelikten, 13’ünün ovarian gebelikten, 8’inin primer abdominal gebelikten ve 5’inin ise bicornuate uterus boynuzundan kaynaklandığını bildirmiştir. Tubal veya ovarian gebelik kesesi rüptürü sonrası fetüs batın boşluğuna geçerek kalsifiye olur (10). Olgumuzda ise tubalarda ve uterusta rüptür bulgusu saptamadığımızdan intra-abdominal yerleşimli lithopedionun ovarian gebelikten kaynaklandığını düşünmekteyiz. Literatürde lithopedion ile ilişkili malignite, pelvic abscess, intestinal obstruction, cephalopelvic disproportion with future pregnancy ve tubal infertility olguları bildirilmiş iken ölümle sonuçlanan olgu henüz bildirilmemiştir (1,3,11). Biz de ölüm sebebini duodenal peptic ulcus perforasyonu ve gelişen komplikasyonları olarak rapor ettiğimiz olgumuzda ölüm sebebini lithopedion ile ilişkilendiremedik.	Translation - English LITHOPEDION INCIDENTALLY DETECTED AS A RESULT OF AUTOPSY FOR AN 87 YEAR-OLD CASE ABSTRACT: Lithopedion is a rare complication characterized by a calcifying dead fetus in intra-abdominal location instead of resorbing. The case is 87 years old and she was transferred to the Morgue Specialization Department of Istanbul Forensic Medicine Institute by the prosecutor's office in April 2014 in order to look into the cause of death. As a result of autopsy, a intra-abdominally located calcified dead fetus of 24-25 weeks according to femur and humerus measurements with a size of 12,5x8x5 cm and weighted 227 gr and a 12-cm calcified cyst in the right ovary was incidentally detected . We were informed that the deceased was married only once with her spouse dying in 1990 and gave 3 births, the most recent of which was in 1946. Since the menopausal age of the case was not known, the retention length of lithopedion was calculated to be 24 to 68 years given the date of the most recent pregnancy and the date of her spouse's death. The case is the first one identified in Turkey and incidentally detected as a result of autopsy, and also one of the oldest cases in literature. INTRODUCTION: Standing for "stone baby" in Greek, Lithopedion is an extremely rare phenomenon associated with calcification of a dead fetus in an advanced ectopic pregnancy. Lithopedion may occur as a result of primary abdominal pregnancy, intrauterine pregnancy resulting in uterus rupture or tubal or ovarian rupture of ectopic pregnancy (1). It can be categorized in 3 subtypes depending on calcification of membranes and fetus: Lithokelyphos where calcification occurs only on membranes, Lithokelyphopedion where both membranes and fetus are calcified, and Lithopedion where only fetus is calcified (2,3). It offers no typical symptoms and there is no diagnostic laboratory test. Cases may be detected through abdominal x-ray usually carried out for other reasons or masses may be felt by hand during abdominal and pelvic examinations. Following a long asymptomatic period of time, it manifests itself with pelvic pain and sense of weight in abdomen. It is recommended to be taken out via surgery as soon as possible after diagnosis since the literature reports that it can lead to acute enteric and urinary system obstruction and even be associated with malignancy (extrauterine placental site trophoblastic tumor) (1,4-6). CASE REPORT: The case was an 87 year-old born woman in March 1927 and reported to die at home in April 2014. According to her daughter, the deceased had lived under the same roof with his mentally-handicapped son, married only once at a young age, and gave three births; one before 1944, one in 1944 and one in 1946, and her spouse died in 1990. Apart from that, we were not informed of whether she had previously any pregnancy resulting in miscarriage or made use of contraception or any time info about her menopause. The case was sent by the prosecutor's office to the Morgue Specialization Department of Istanbul Forensic Science Institute to perform autopsy for the cause of death due to the fact that she had been found dead even though she was not suffering from an apparent disorder at the time and there was not anything suspicious at home. At the autopsy, external examination pointed to poor personal care and hygiene with a thin body as well as local scratches and ecchymoses on both elbows and on dorsum of feet. Radiological imaging revealed a calcified fetus image in the bottom right-hand of abdomen, and a round-shaped cystic membrane calcified in pelvis (Fig.1) Figure 1: a; Calcified cystic membrane in pelvis, b; Lithopedion In the abdominal cavity, we came across with 700 mL malodorous, yellow-brown color infected fluid and peritoneal findings. On the bottom-right hand of the abdomen, a calcified fetus was turned out to be attached at the lower extremities-level, right under omentum majus. No placenta or other pregnancy-related signs were observed. Figure 2: Intra-abdominal lithopedion Calcified fetus was 12.5x8x5 cm in size while femur length was 4.9 cm and humerus length was 4.6 cm and it weighted 227 grams (Fig.3). Figure 2: Lithopedion aspects According to our measurement, uterus height was 10.5 cm, uterus width was 10 cm, myometrium thickness was 2 cm, left ovary was 5.5x5x4.5 cm in size, and right ovary was 5.5x4x4.5 cm. Also, we detected a calcified cyst with 12 cm in diameter in right ovary. Uterus and tubes were sound, and we detected no rupture. We detected 0.5 cm-long hemorrhagic peptic ulcus perforation around 1 cm distal of pylorus on duodenum. We also detected findings suggesting chronic heart failure along with acute myocardial infarction. Other chest and abdominal organs and the brain offered no findings other than those that fit for her geriatric age. Chemical examination pointed to therapautical-level Naproxen. Histopathological test indicated a recent myocardial infarction (fitting for the first 24 hours), a myocardial scar area, fibrin thrombus on some medium and large-scale vascular lumens of lung, autolysis and erosive changes in duodenum mucosa, and peritoneal findings in serosa and omentum. In conclusion, we reported that the death of cause for the deceased was duodenal peptic ulcus perforation and related complications (peritonitis, acute myocardial infraction). DISCUSSION: Lithopedion is defined as a calcified dead fetus with intra-abdominal location. It may occur due to primary intra-abdominal pregnancy or secondary abdominal pregnancy as a result of rupture with intrauterine or tubal location (7). The progress of lithopedion requires the extra-uterine located fetus to survive for at least 3 months and remain sterile, medically not diagnosed or undiagnosed while favorable conditions must be available for calcification (2). In literature, there are reportedly around 300 cases of the stone baby phenomenon (8). It is an incidentally detected rare phenomenon that occurs for 1.5 to 2 % of ectopic pregnancies and 0.0054 % of all pregnancies (3). It is typical for people with poor socio-economic conditions and insufficient prenatal care (1,2). This case had 3 children and lived along with her mentally-handicapped son. While the information provided by her daughter are not satisfactory, we think that living conditions of the deceased were not decent as she had poor personal care and lived along with her mentally-handicapped son. In literature, the age of such cases ranges from 20 to 100 while two-thirds of the cases are above 40 years of age. Retention time of Lithopedion ranges from 4 to 65 years (1,7,8). This case was one of the oldest ones (87 years old) in literature and found dead at home. Since we do not know about the menopausal age of the case, it is safe to calculate the retention length for lithopedion as 24 years at least and 68 years at most given the fact that the most recent birth she gave was in 1946 and her spouse died in 1990. Fetal death occurs between the 3rd and 6th month for 20% pregnancies, between the 7th and 8th month for 27% of pregnancies, and in full term for 43% of them (1). Lithopedion of the case was 12.5x8x5 cm in size, weighted at 227 gr while fetal death occurred between the 25th and 29th week according to femur and humerus measurements (9). Tien collected 114 cases of lithopedion and reported that 74 of them were caused by tubal pregnancy while 13 were caused by ovarian pregnancy. 8 of the cases were induced by primary abdominal pregnancy while 5 were caused by bicornaute uterus (horn). Fetus makes its way to the abdominal cavity and gets calcified following rupture of gestational sac in tubal or ovarian pregnancy (10). This case, on the other hand, offered no rupture in ducts and uterus. Therefore, we think that the intra-abdominal located lithopedion results from ovarian pregnancy. The literature reports lithopedion-induced malignancy, pelvic abscess, intestinal obstruction, cephalopelvic disproportion with future pregnancy and tubal infertility cases while there has been no case reported to result in death (1,3,11). We could not associate the cause of death to lithopedion for this case who died as a result of duodenal peptic ulcus perforation and related complications.

Source text - Turkish 1985 doğumlu gebe, 16.05.2007 tarihinde özel bir hastanede doğum yaparak, bir kız çocuğu dünyaya getirmiştir. Anne, hastanedeyken bebeğini emzirmek için yanına getirdiklerinde topuğunda pamuk gördüğünü, fakat zeka geriliği (FKU testi) ile ilgili kan alınıp alınmadığını kendilerine söylemediklerini ifade etmiştir. Anne ve bebeği 17.05.2007 tarihinde taburcu edilmiştir. Anne taburcu edildikten 2 gün sonra bayılmaları nedeniyle hastaneye başvurduğunu ve bu sırada bebekten zeka geriliğinin tespiti için kan aldıklarını belirtmiştir. 26.07.2007 tarihinde (bebeğin doğumundan 2 ay 10 gün sonra) bebek aşı için sağlık ocağına götürüldüğünde, Neonatal Tarama ile ilgili kanın alınsa bile aileye geri bildirim yapılmadığının anlaşılması sonrasında bebeğin topuğundan bu amaçla topuk kanı alınmıştır. Burada alınan topuk kanının sonucu 17.09.2007 tarihinde sağlık ocağına faksla bildirilmiştir. Sağlık ocağı tarafından ailenin evine yapılan ziyarette topuk kanı sonucu FKU (+) yerine TSH(+) olarak belirtilmiştir. Aile Cerrahpaşa Tıp Fakültesi’ne yönlendirilmiş ve burada bebekte TSH ile ilgili tetkiklerin yapılması sonucunda TSH normal olarak çıkmıştır. 02.10.2007 tarihinde TSH sonucu aile tarafından aynı gün sağlık ocağına bildirilmiştir. Sağlık ocağı tarafından İl Sağlık Müdürlüğü’ne normal olan TSH sonucunun bildirilmesi üzerine, müdürlüğün TSH’ın değil FKU’nun alınması gerektiğini uyarması sonucunda 10.10.2007 tarihinde aileye tekrar ulaşılmıştır. Ailenin hastaneye gitmemesi üzerine 25.10.2007 tarihinde tekrar aileyle irtibata geçilmiştir. Aile araya bayramın da girmesi nedeniyle 01.11.2007 tarihinde tekrar Cerrahpaşa Tıp Fakültesi’ne başvurmuş ve FKU (+) üzerine Çocuk Metabolizma bölümüne sevk edilerek bebeğin diyeti ve tedavisi düzenlenmiştir. Bebeğin doğumundan 3 gün sonra alınan topuk kanının kayıtları kontrol edildiğinde bilgisayar arızası nedeniyle herhangi bir kayıt bulunamamış ve kanın Sağlık Grup Başkanlığı’na iletilmediği saptanmıştır. Bunun yanında bebeğin doğum raporunun da Sağlık Grup Başkanlığı’na iletilmediği görülmüştür. Yüksek Sağlık Şurası’nın 3-4 Mart 2012 tarihli raporunuda; bebeğin doğumundan sorumlu kadın doğum doktoru, hastayı taburcu eden hemşire (kimliği anlaşılamamıştır), sağlık ocağına gelen sonuç faksının yanlış okuyan personel, tıbbi tahlillleri yapmakla yükümlü firma sonucu 72 saatte tespit etmemişse firmanın ve süreci yönetemeyen hastane idaresinin birlikte kusurlu olduğu oy birliği ile karara bağlanmıştır.	Translation - English Born in 1985, the expectant mother gave a birth to a baby girl in a private hospital on May 16, 2007. The mother pointed out that she saw cotton on her baby's heel once they brought the baby for her to breastfeed whereas she was not told whether any blood was drawn to perform a mental retardation test (PKU test). The mother and her baby were discharged on May 17, 2007. 2 days after she was discharged, the mother reportedly resorted to a hospital due to fainting and said that blood was drawn from her baby to perform the mental retardation test. When the baby was taken to a primary healthcare center on July 26, 2007 (2 months and 10 days after her birth), some blood were drawn from the baby's heel once it was concluded that the family had not been provided with any feedback even though blood was drawn for a Neonatal Screening. The result of the test performed on the blood taken was submitted to the primary healthcare center on September 17, 2007. Upon a visit paid by the primary health center to the family's residence, the result of the test for heel blood was reported as TSH (+) instead of PKU (+). The family was referred to Cerrahpaşa Faculty of Medicine and then TSH turned out to be normal following the examination performed on the baby for TSH. On October 2, 2007, the result of TSH test was notified by the family to the primary healthcare center. Upon the primary healthcare center submitted the result of TSH test, which was normal, to the Provincial Health Directorate, the family was reached out once again on October 10, 2007 as a result of a warning made by the Directorate saying that it was PKU, not TSH that a test should have been performed for. The family was contacted again on October 25, 2007 as they did not go to any hospital. Due to a nationally-held religious holiday, the family waited until November 1, 2007 to apply to Cerrahpaşa Faculty of Medicine and the baby's diet and treatment were put in order after the PKU test had resulted in (+) following her referral to the department of pediatric metabolism. When the records for the blood drawn from the baby's heel 3 days after her birth were checked, no record was found due to a computer-related failure and then it was found out that the blood was never submitted to the Healthcare Group Department. In addition, it was reported that the baby's birth certificate was never submitted to the Healthcare Group Department, either. In the wake of an investigation launched by an expert assigned following this incidence, it was concluded that the private hospital never submitted the blood to the Healthcare Group Department even if they drew blood, and that the Department never took delivery of the birth certificate, and that the failure of the hospital to submit the blood and the birth certificate led to a delay in diagnosis of the disease and that the primary healthcare center reported the result of the heel blood test as PKU (+), and that the loss of time caused by wrongly assuming that the result was TSH (+) instead of PKU would not change the end result. In a report released on March 3-4, 2012 by the Supreme Council of Health, it was unanimously decided that the female obstetrician responsible for the baby's birth, the nurse who discharged the patient (not identified), the staff member who misread the fax submitted to the primary healthcare center for the result of the test, the company responsible for performing the medical tests if it could not establish the result of the test in 72 hours, and the hospital administration were all culpable.

The baby was brought to the same hospital for routine checks on August 18, 2004 and as a result of the screening tests, the PKU turned out to be 0.9 and the result was normal. The baby was then diagnosed with Phenylketonuria. General practices require to perform a screening test for phenylketonuria between the day 5 and the day 7 for all newborns. According to the medical documents and hospital records, the baby was reportedly tested for the phenylketonuria level on day 5 after the birth. Since the result was normal, Dr. Kürşat Özer was asked whether the test, which was reportedly performed in the hospital, was actually performed and whether it was performed duly. In case it was performed duly, then the hospital could not be impeached as it was unanimously thought that the results of screening tests would be rarely wrong or incomplete. On August 13, 2004, the baby was born at Private Erdem Hospital weighing 2500 gram with 46 cm in length, and 33 cm in head circumference and was examined within the first 24 hours following the birth and discharged since no problem was detected. On August 18, 2004 that is 5 days after the birth, the baby was brought back in the hospital for routine checks and weighed 2260 gr (9.6% weight loss) and the baby's entire body was showing signs of jaundice. Some blood were drawn from the baby for total bilirubin and screening tests (TSH and PKU). The screening tests (TSH Neonatal: 5,5 ml/ MI and PKU: 0,9 mg/dl) turned out to be normal. In consideration of information and findings specified in judicial and medical documents drawn up for Hümeyra Hanilçe who was afterwards diagnosed with Phenylketonuria, in another health facility: 1- The report released on October 26, 2011 by the the Council of Forensic Medicine that points out that "the baby was checked for the phenylketonuria level on day 5 after the birth. Since the result turned out to be normal, Dr. Kürşat Özer was asked whether the test, which was reportedly performed in the hospital, was actually performed and whether it was performed duly. In case it was performed duly, then the hospital could not be impeached as it was unanimously thought that the results of screening tests would be rarely wrong or incomplete is unerring. 2- The guideline for the kit (Neonatal Phenylalanine Kit) was reviewed by our party as it was included in the file and reported to be routinely used in the hospital on that day, and it was concluded that the kit was put into use to identify quantitative Phenylalanine from a blood sample dried on a filter paper in an effort to determine high Phenylalanine levels for newborns, and the kit was an analytical technique acknowledged and administered under modern conditions. 3- As it is evident from the circular letter of the Ministry of Health numbered 2006/130, it is not an obligation to submit blood drawn from heels to the Healthcare Directorates prior to December 25, 2006, and that was why it was not possible to consider the failure of the hospital to submit the heel blood to the relevant Health Directorate as a negligence. 4- It was concluded from the information available that there was no judicial or medical proof that that test was not performed or it was fake. However, in case a need arises for the Supreme Court, as expressed by Dr. Halil Köksal: "We have computer records. No one can doctor computer records for results of laboratory tests. The computer records the date when it is intervened by any alteration or falsification. This is something that can be revealed. If required, one can review the computer records." It is concluded that computer forensics experts can launch an investigation to identify when the Phenylalanine test that was allegedly performed for Hümeyra Hanilçe was recorded on the computer or whether the results that had been allegedly recorded on the computer were altered or not.

Anne ve bebek bu hastanede 2 günü aşkın bir süreyle tıbbi bakım altında kalmışlar ve ardından 17.05.2006 tarihinde taburcu edilmişlerdir. Bu süre zarfında bebekten FKU testi için topuk kanı alınmadığı gibi taburcu edilmeden önce de kan alınmamış ve gereken testler yapılmamıştır. Daha sonra aile bebeği 22.05.2006 tarihinde, doğumdan sonraki 7. günde bir başka özel hastaneye götürerek kontrolünü yaptırmışlardır. Burada, muhtemelen doğumun başka bir hastenede gerçekleşmiş ve doğumun üzerinden 7 gün geçmiş olması nedeniyle topuk kanı alınıp alınmadığı ve alındıysa sonucun ne olduğu sorgulanmamıştır. Literatürde yer alan bilgilere göre topuk kanı alınması için önerilen ideal zaman dilimi 24-48 saat aralığı olup, bebeğin 24 saatten önce taburcu edilmesi halinde topuk kanının taburcu edilirken en son anda alınması, erken taburcu olması nedeniyle ilk 24 saat içinde alınması gerekmiş ise 5. – 25. günlerde tekrar edilmesi, ilk kan alımının 7 günü geçmemesi önerilmektedir. Ancak bu noktada bebek 35 gün içerisinde 3 kez ana çocuk sağlığı ve aile planlaması merkezine aşı ve kontrol için götürülmüştür. Yine doğumun gerçekleştiği hastanede sağlık personeli olarak çalışan bir kişinin verdiği ifadeye göre doğum yapan anneye kontrol tarihi verilmiş ve çocuğun kontrolü için ikinci özel hastanedeki bir doktora yönlendirildiği, kan tahlili için ise kendi hastanelerine çağırdıkları beyan edilmiştir. Ayrıca bebek kontrollere düzenli aralıklara getirilmemiş, ilk kontrolün ardından 1., 5., ve 8. Ayda kontrole getirilmiştir. Fenilketonüri gelişimi yavaş olup gelişme geriliği ile ve özellikle de zeka geriliği ile ancak bu gelişimin izlenebileceği aylardan sonra klinik tanı konabildiği bilimsel kaynaklarda belirtilmektedir. Erken tanı için önemli kriterlerden biri olan kafa çapı gelişiminde geriliğin doktor tarafından bebek 8. Ayda muayeneye getirildiğinde fark edildiği ve gerekli inceleme için sevk edildiği, küçüğün hastaneye de bu tarihten 2 ay sonra götürüldüğü kayıtlardan anlaşılmaktadır. Translation - English On May 15, 2006, a baby boy was born through cesarean section at a private hospital. The mother and the baby were put under medical care for over 2 days at the hospital and discharged on May 17, 2006. Over this period of time, no blood was drawn from the baby's heel for a PKU test and the required tests were not performed, either. On May 22, 2006, the family of the baby took him to another private hospital on day 7 after the birth and had him checked. The hospital never questioned whether heel blood was ever drawn and what the result was if it had been probably due to the fact that the birth was given at another hospital and 7 days went by since the birth. In accordance with the information in literature, the ideal time frame to drawn heel blood ranges from 24 to 48 hours while it is recommended to draw heel blood at the last second while a baby is discharged in case he/she is discharged before 24 hours are over, to repeat the procedure on day 5 to 25 if it is necessary to draw heel blood within the first 24 hours due to an early discharge, and to make sure that the first blood draw is not after day 7. In the meantime, the hospital officials submitted the birth certificate along with his current medical status to the Healthcare Group Department of the District Governor's Office. The Healthcare Group Department, then, notified the Maternal and Child Care and Family Planning Center of the situation for further action. The Center argues that they stopped by the family's residence but they could not find any family member. However, the baby was taken to the maternal and child care and family planning center for 3 times within 35 days for immunization and medical exams. According to the statement of a healthcare professional working for the hospital where the birth was given, the mother was notified of a date for medical exams and referred to a second private hospital for the baby's medical exams, and then they were called by the hospital to stop by for blood tests. In addition, the baby was not brought to the hospital for medical checks on a routine basis. He was brought to the hospital for medical exams only on month 1, 5 and 8. Phenylketonuria develops slowly while scientific sources point out that it can be clinically diagnosed following the months when this development can be monitored with regard to growth delay and mental retardation in particular. It is concluded from the records that retardation in head circumference growth was identified by the physician when the baby was brought for examination on Month 8, and then he was referred for necessary procedures, and brought back to the hospital 2 months later. As pointed out in the Circular Letter of the Ministry of Health and the report issued by the panel of experts regarding the action for damages, the circular obligates to draw heel blood at the last second "before a baby is discharged from a healthcare institution” and the hospital where the birth was given did not fulfill this responsibility. In consideration of the Legislation, a screening program was launched for Phenylketonuria in 1987 in 22 provinces and in 1993 across Turkey and it is still being performed successfully. Since December 25, 2006, all the hospitals and healthcare institutions across Turkey have incorporated the Congenital Hypothyroid to the Phenylketonuria Screening Program and therefore the Maternal and Child Care Center is also responsible in this regard as the birth specified in the report issued by the panel of experts regarding the action for damages was given prior to December 25, 2006. In addition, it would not be a proper self-defense for the Maternal and Child Care Center to claim that they could not reach out to the family. The family also needs to be held responsible for failing to comply with the recommendation to have a blood test performed with regard to the development of Phenylketonuria. However, the fact that the physician working for the hospital where the baby was taken to for routine exams reviewed the patient's file gives rise to the thought that he/she did not properly do what was necessary in terms of medical requirements as he/she was supposed to make inquiries once he/she could not find any PKU test performed.
