Matt, the sentence "ficar com fome" could be what people say after a meal at which they didn't eat enough as in: "ainda fiquei com fome" (to still be hungry)
It could also be what you say when you are starting to feel hungry, as in: "estou a ficar com fome" (I am getting hungry).

I think this covers all the bases! It simply means to be hungry - the other mutations would be appropriate depending on the context, I disagrre with 'get hungry' because that pertains to active change, whereas ficar does not: ficar highlights a state/condition, not a change/movement/action. Ficar com fome, stricktly speaking can not mean to get hungry, since ficar, by definition, is not an action per se....
