이후 동의서의 Main version 변경 (영문 피험자 설명문 및 동의서 version 02_23Feb2009, 국문Version02_05Mar2009) 신청이 제출되어 2009
년 5월 29일에 동의서 변경을 승인받습니다 (당시 추가 요약본은 수정하여 제출되지 않고 누락되었습니다).
이후 Main version 2로 변경된 이후에 모집된 피험자(45170004~45170015)에 대해서는 version 02 이후에 IRB에 제출하고 승인받은 동의서
(국/영문 ICF main version 02, 03, 04)에 대해서는 추가 요약본이 아닌 전체 동의서를 이용하여 설명한 후 동의를 득하였습니다.

On the initial review in this study in 2008, the Boramae medical center’s IRB opinion was ‘Please provide additional summary written in simple terms as the ICF provided to subjects runs to 17 pages including a number of technical terms’. According to the opinion, the ICF submitted at the initial review (English version 01_08 Oct 2008 and Korean version 01_31 Oct 2008)had been simplified to 6 page-long Korean/English subject information sheet and ICF supplementary summary(version 01_29Jan2009). And it had been submitted and approved.
Subject number 45170001,45170002,45170003 were provided and explained about both the full ICF and the supplementary summary and those subjects signed the ICF. After that change of main version ICF(English subject information sheet and ICF version 02_23Feb2009, Korean Version02_05Mar2009)application was submitted and approved in May 29,2009 (Supplementary summary was not submitted with corrections and then it was excluded at the time). After that, the subjects after changed Main version 2(45170004~45170015) were obtained the ICF through using not the supplementary summary but the full ICF. In that process, in regard to the ICF which was submitted and approved after version 02, it was used not the supplementary summary but the full ICF. Thus it is reported to the IRB.
